| Inspection of the well showed 8 5/8 surface casing with 4 1/2 production casing with a cap on the production casing. Division records show two completion records. The first report dated 6-4-1974 states the well was never perforated or fracked. The second report, dated 6-14-1974, is a Geo Survey card that states Frac A.F. 550MCFG, Show oil, gas Nat., perforation information is not on the card. This is a rotary drilled Clinton well with cemented in surface and production casings, see division records. The visible production casing is full of holes and the remaining exposed steel is very thin. The production casing is almost completely rusted through at the intersection of the production and surface casings. Fluid is inside the production casing, level with the surface casing intersection. Quantab 1176 peaked at greater than 6,000 ppm chloride inside this fluid inside the production casing. There is not evident bubbling or oil coming through the fluid but the fluid occasionally overflows onto the ground on the west side of the well. A 35 x 65 area 20 west of the well is void of vegetation. Quantab 1176 peaked at greater than 6,000 ppm chloride in a puddle of water in this area. It is not possible to check for fluid between the surface and production casing annulus during this inspection due to the surface casing head. A gas odor is evident at the well. My Sperian gas scope showed 4 % LEL/ 0 ppm H2S at the holes in the production casing. The surface casing head is rusty and the steel is flaking off in pieces. The head is equipped with two openings, one opening is equipped with a plug. The second opening is equipped with a nipple. This nipple has a hole in it before the valve and my Sperian gas scope showed 100% LEL/0 ppm H2S at this spot. My Sperian gas scope showed 4% LEL/0 ppm H2S at the open valve. There is not a tank battery or lease road affiliated with this well. The landowner/complainant, Mr. Duplay, stated the well was never put into production. Giant Energy in

| Todays inspection was conducted to check the condition of the well and the well site. This well is currently enrolled in the Orphan Well program for plugging. The 4.5 inch diameter production casing is more deteriorated than last inspection and is almost rotted off at the intersection of the 8.65 inch well head. Gas is still leaking from the hole in 8.65 inch nipple.

| Todays inspection was conducted to measure areas that are void of or have distressed vegetation and to determine possible routes into the site from State Route 534. An approximate 65 foot long x 25 foot wide, east to west, stressed area is located north of the well. Fluid with a chloride content of over 6,500 pppm occasionally overflows from the open 4.5 inch casing into this area. The 4.5 inch casing has deteriorated to the point of almost falling over at the intersection of the casing and 8.65 inch well head.

| On site with Orphan Well Manager, Gene Chini, to evaluate the condition of the well for possible plugging by the Orphan Well Program. See earlier inspection report.  Fluid is evident inside the production casing. The exposed production casing has deteriorated and is full of holes.  The fluid inside the production casing occasionally overflows the west side of the surface casing. The Quantab peaked at over 6500 ppm Chloride when placed in the fluid. The gas leak through the hole in the 8 5/8 nipple is stronger then observed at the last inspection and is blowing under pressure. Rain this morning has filled the deer tracks in the area void of vegetation located west of the well in the corn field. The high range Quantab showed chlorides at approximately 3800 ppm in the water in the deer tracks. There is not evident oil and gas coming through the fluid in the production casing. A chiefs order has been submitted to Columbus for review for possible bond forfeiture. Giant Energy Corp. is no longer in existence.

1496253897 2017-05-18 Orphan No Violations Todays inspection was conducted to check the condition of the well and the well site. This well is currently enrolled in the Orphan Well program for plugging. The 4.5 inch diameter production casing is more deteriorated than last inspection and is almost rotted off at the intersection of the 8.65 inch well head. Gas is still leaking from the hole in 8.65 inch nipple.
1502210442 2017-07-27 Orphan No Violations Todays inspection was conducted to measure areas that are void of or have distressed vegetation and to determine possible routes into the site from State Route 534.
1505757127 2017-09-13 Orphan No Violations Todays site visit was to attend a pre-bid meeting held by the Orphan Well Program for contractors to preview the site and ask questions prior to submitting plugging bids. Mr. Duplay, the landowner, was on the property but did not attend the meeting.

1955420749 2015-08-18 Orphan No Violations On site with Orphan Well Manager, Gene Chini, to evaluate the condition of the well for possible plugging by the Orphan Well Program.
