| During a status check inspection, the 210 bbl stock tank was found to have holes approximately six feet above ground level and was seeping oil and incapable of containing product. The 210 bbl tank is in very poor condition and needs to be properly repaired or replaced. There was also no identification at the wellhead. The three Rubber City wells have a common tank battery with three 210 bbl tanks, one 100 bbl tank and three horizontal separators. I contacted Dan Long (330-866-3558), the pumper for Cutter and informed him of the problems.

| The Rubber City Sand and Gravel # 3 well was drilled and completed in 2/1987. A review of the RBDMS data base indicates that the well is current in the reporting of the production volumes. The well construction is 8.625 surface casing is set at 505, 4.5 production casing set at 4354 and 1.5 tubing set at 4170. The well is operated as a plunger lift well on a Logic controller. The required identifying signage, at the wellhead, does not reflect the current Operator. The fencing around the wellhead is in need of repair and a lock to maintain security. The well location is located in a high density residential area and the landowner allowed that he has incidents with trespassers previously. There are numerous leaks on the wellhead. The Demco gate valve ( 1.5 tubing ) is leaking and the top joint of 1.5 tubing appears to be also leaking. These leaks are running down the tubing head and 8.625 casing head to the ground. The motor valve is also leaking but is not causing ground contamination at this time. The 8.625 casing head has two ( 2) outlets opened to the atmosphere. There are not any valves or bull plugs in the outlets. The tank battery is shared with the Rubber City Sand and Gravel # 2 ( 34-153-2-2250, Skyliner Energy ) . The Rubber City Sand and Gravel # 3 well  has a 100 barrel oil tank and a 2408 mechanical separator. The tank battery was fenced, locked and secured. The 100 barrel oil tank had the required identifying signage reflecting the current Operator. I did not identify any leaks or violations associated with the tank battery for the Rubber City Sand and Gravel # 3 well. A coated gas sales line to Dom / EOG is on top of the ground, unburied for approximately 30 heading east from the tank battery toward Myersville Road. In order to bring the Rubber City Sand and Gravel # 3 well into compliance the Operator shall : 1.) Post the required identifying signage at the wellhead within two (2) weeks of receipt of notice - OAC 1501:9-9-05 ( 10 ) ,  2.) Operator shal

| My inspection found the well is has partially complied with the Compliance Notice issued on 01-26-2016.  The well is equipped with a plunger lift system and it is not leaking.  The well is overgrown with woody-stemmed vegetation making it difficult to access.  The tank battery consists of a steel production tank, a horizontal separator and a meter.  The well is legibly identified.  The coated section of sales line hs not been buried.  I called 330-897-1899 there was no answer and  I was unable to leave a message.
