He also said that efforts will be made to remedy the problem, but it could be a combination of rain and the fact that the tank battery is in a low lying area. Neither of the tanks have been losing fluid. Clean-up has been performed around the well head. The leaky valve has also been fixed.
0216973396 2009-04-29 Administrative Inspection No Violations Contacted Mr. Ron McBride at 1:36 PM, Chieftain Energy Contractor, on cell phone, and was instructed to repair the earthen dike around the tank battery. Quantabs had shown chlorides inside and outside the earthen dike. Mr. McBride was given two options for contaminated soil: remove contaminated soil, new topsoil, seed and mulch or bioremediation to include fertilizer, lime, seed and mulch. Mr. McBride agreed to repair the dike and has opted to perform bioremediation.

0420304997 2011-10-05 Production Wells Violations Noted A follow up inspection revealed that there was a flow line leaking gas and blowing bubbles into a pool of fluid outside of the dike. The kill zone caused the pool of fluid was approximately 12 by 35. The fluid was tested with High Range Quantab titrators it registered 8.6 which is 5912 ppm (mg/L) chloride. There was no oil sheen on the fluid. There was also an abundance of fluid inside of the dike. Met Ron McBride, and Doug Kitchen in the field. They attempted to fix the leak, but werent able to, so they shut the valve and will fix it when the clean up of the contaminated material takes place. Ron and Doug were given a week to remove contaminated soil, and get dike sucked out. If complied with by the extension date, company may take contaminated soil back to their shop for bioremediation. If not, contaminated soil will have to be disposed of in an EPA approved landfill.
0510898090 2012-11-05 Production Wells No Violations FOLLOW UP TO NOV # 1352148423 Received phone call from Mike Sherron (OEPA) on Sunday 11/4/12 at 5:45 pm stating that the dike for the tank battery had over flown, there was a valve bubbling gas inside the dike, there was a slight oil sheen and that he had deployed an absorbent boom across the point of escape. When I arrived on site I observed the dike full of fluid, and the boom deployed. There was a very slight oil sheen, and small amounts of residual oil on top of the water. The boom had only thin stripes of contamination on it. The point of escape for the water was a low place in the dike that hadnt been completely closed after work had been done on the tank battery. The valve was that was reported as bubbling was indeed bubbling. I tested the water for chlorides in three places: inside the dike, just outside the dike, and in a pool of water 20 yards away. The water inside the dike registered a 4.2 on the High Range Quantab which means there were 787 parts per million chlorides in it. Both quantabs on the outside of the dike did not register enough chlorides to quantify. I spoke with Mr. Bill Wray, explained to him the situation, and gave him instructions to empty the dike of any fluid. He stated that it would be done this evening. As far as fixing the bubbling valve and the low spot on the dike, that would be taken care of later this week. I also brought to his attention remedial clean-up needed around the well head. He stated that that will also be taken care of this week. Photos were taken at the time of inspection. Spoke with property owner Lowell Nutter during inspection.

1155927171 2006-08-09 Production Wells Violations Noted An inspection was conducted at the facility owned by Chieftain Energy Corp on 8/9/2006. During the inspection, it was noted that the well utilized a common tank battery and no ID was observed at the well. Also noted was that the oily soil needs to be removed from the lease.
1239281417 2009-03-18 Production Wells Violations Noted An inspection revealed contamination and pollution near the well and inside the earthen dike. The dike has reached maximum capacity of fluid and was also found to contain oil. The source of the leak is from a separator inside the dike and two fittings on the bottom of (2) separate 100 bbl tanks showed signs of leakage (stains). The tank battery is located in a low lying area at the edge of a pasture field, and road located approximately 300 feet away from a nearby creek. At the time of the inspection, the area surrounding the well and tank battery location remain wet, due to rainfall events, and runoff from nearby road. The earthen dike may be leaking and will be evaluated when the surrounding area begins to dry. Quantabs (High Range) placed at locations showed no signs of accelerated chloride. No oil sheen was observed outside the dike. Near the well location, old parrafin was observed on the ground surface and will need to be removed. Note: The company was informed of the inspection results, in the field, and also by telephone to include that the dike may be leaking and will be evaluated.
1280009483 2009-04-27 Production Wells Violations Noted Chieftain Energy Contractor, Mr. Ron McBride called by telephone today and stated that the dike had been pumped. A follow-up inspection revealed that the dike has been pumped, but contamination and pollution still remains in the dike. The separator still needs to be repaired, due to a bad dump valve. Due to a lack of rainfall events, the earthen dike was again evaluated to ensure that it was not leaking. During the inspection, it was noted that the dike would need to be repaired. High Range Quantabs (Titrators for Chloride) were utilized at three locations. Two locations inside the dike measure 2.8 and 3.8 units, which equals 538 and 899 ppm (mg/l) Cl- respectively. A location behind the dike contained a bare area with deer tracks measuring approximately 20 x 100. A titrator placed near the beginning edge of this bare area showed a value of 4.2 units, 1079 ppm (mg/l) Cl- . Note: A nearby creek is approximately 300 from this location. The trench along the edge of the field allows water to drain offsite and into the creek. A titrator placed near. Chieftain Energy will be contacted regarding this inspection.
1302859697 2010-12-03 Production Wells No Violations An inspection revealed small gas leak around polish rod liner. The liner leans to one side, as pumping unit does not appear to be level. Valves were observed open and well pumps into a shared tank battery. At time of the inspection, no statements of production have been submitted to the Division since 2006. Chieftain Energy Corp. is currently addressing this deficiency for the years 2007-2009 on all wells.
1302906523 2009-05-11 Production Wells No Violations A follow-up inspection revealed that dike drain was releasing fluid from the dike. Oil was observed inside the dike. Outside the dike a 5 x 3 section of oil was observed. Quantabs showed acceptable amount of Cl- in fluid. Contacted Chieftain Energy and instructed to install dike and apply absorbent pads to areas that contain contamination and pollution. The equipment that had previously been observed has now been removed from the location. The dike does not show signs of being repaired only installed drain.
1312898085 2011-08-02 Production Wells Violations Noted An inspection of the tank battery and separator revealed two violations. There was water in the tank battery that when tested with high range Quantab titrators, completely maxed them out. The chloride content is greater than 6765 ppm (mg/L). Water needs to be removed, and the soil underneath also needs to be removed and hauled away. Gas was being vented from a pop-off valve on top of the separator. This needs to be corrected, or a flare needs to be constructed as per OAC 1501:9-9-05(B). Property owner receives house gas.

According to the owner of Chieftain Energy, Mr. Doug Kitchen, and the landowner, Mr. Lowell Nutter all seven wells are tied into each other to produce house gas. Mr. Nutter states that little gas is obtained from these wells. Mr. Kitchen admitted that a few wells need some work. Mr. Kitchen will be given credit for 1 well since some house gas is generated. The other 6 wells must be plugged or produced. Mr. Kitchen was given a verbal NOV on 10/18/2006.

Contaminated soil had been removed and stockpiled on location. Mr. Kitchen and Mr. Nutter agreed on composting contaminated soil where it was stockpiled. Mr. Nutter offered this as a solution. Contaminated soil will be remediated by mixing horse or cow manure and lime with it and will be re-mixed approximately 6 months from now.
