| On 7/18/2012, I conducted a production well inspection status check.  My inspection found the well was a plunger lift unit (rabbit) that was not producing, evident by both ball valves in the closed position.  The tank battery was overgrown with weeds and contained one 210 barrel oil storage tank, with no identification posted, a buried concrete brine tank and a separator.  I noticed an oil spill next to the brine tank about 4' x 2' and an oil stains between the oil tank and brine tank.  I took photos of the oil spill, the tank battery, and the wellhead.  The lease road came off the driveway, but was overgrown with grass.  I called David Waldron & Associates and found out the well belonged to the late David Waldron and was included in his estate, which was being handled by Attorney Bill Ross.  I called Bill Ross at 330-264-1213 and left a message to call me to discuss this well site.

| I performed an administrative inspection in reference to compliance notice #1344513903.  I called Attorney Stork to inquire on the status of well.  Attorney Stork claimed a deed was located which stated the well was transferred to the landowner in 1990.  Attorney Stork stated he was trying to contact the new Amish landowner through the mail to find out if they would be interested in signing a Form 7 to own the well.  I instructed Attorney Stork to contact ODNR in Columbus to verify that the transfer on the deed was legal prior to having the new landowner sign any transfer document.  I will continue follow-up inspections.

| I was on location for a domestic well inspection.  During my inspection, I found the well being used for domestic gas.  The well was equipped with a plunger lift assembly.  There was a 210 barrel storage tank, a buried, concrete brine tank, and a vertical separator.  A dike surrounded the production equipment.  The flow line ball valve leading from the well to the production equipment was in the closed position.  I spoke with Mrs. Byler concerning her responsibilities and the requirements for an exempt domestic well.  I found that:

0522471051 2013-12-16 Production Wells No Violations My inspection found the identification still has not been posted. The well was shut-in; however, the domestic side was in use. I contacted Attorney Stork at 330-263-0006 to get an update on the well. Attorney Stork was working on either transferring the well to the landowner, selling the well to another company, or plug the well. I will continue follow-up inspection.
0642191746 2014-05-21 Administrative Inspection No Violations I performed an administrative inspection in reference to compliance notice #1344513903. I called Attorney Stork to inquire on the status of well. Attorney Stork claimed a deed was located which stated the well was transferred to the landowner in 1990. Attorney Stork stated he was trying to contact the new Amish landowner through the mail to find out if they would be interested in signing a Form 7 to own the well. I instructed Attorney Stork to contact ODNR in Columbus to verify that the transfer on the deed was legal prior to having the new landowner sign any transfer document. I will continue follow-up inspections.
1344513903 2012-08-09 Production Wells Violations Noted On 8/9/2012, I conducted a follow-up inspection of the oil/brine leaks. My inspection found all the contaminated soil and grass has been removed and replaced by clean top soil. The proper identification has not been posted. I called Attorney Bill Ross at 330-264-1213 to have the proper identification posted, who cleaned the oil/brine spill, and if the well was going to be plugged or sold to the landower for domestic use.

I called Attorney Bill Ross and left a message for the proper identification sign to be posted. Also, I told him the well needed to produce, file for a plugging permit, or sell the well for domestic use. I will continue follow-up inspections.
1739469264 2012-07-18 Production Wells Violations Noted On 7/18/2012, I conducted a production well inspection status check. My inspection found the well was a plunger lift unit (rabbit) that was not producing, evident by both ball valves in the closed position. The tank battery was overgrown with weeds and contained one 210 barrel oil storage tank, with no identification posted, a buried concrete brine tank and a separator. I noticed an oil spill next to the brine tank about 4 x 2 and an oil stains between the oil tank and brine tank. I took photos of the oil spill, the tank battery, and the wellhead. The lease road came off the driveway, but was overgrown with grass. I called David Waldron & Associates and found out the well belonged to the late David Waldron and was included in his estate, which was being handled by Attorney Bill Ross. I called Bill Ross at 330-264-1213 and left a message to call me to discuss this well site.
1831879087 2014-11-20 Exempt Well Pre Transfer No Violations I was on location for a domestic well inspection. During my inspection, I found the well being used for domestic gas. The well was equipped with a plunger lift assembly. There was a 210 barrel storage tank, a buried, concrete brine tank, and a vertical separator. A dike surrounded the production equipment. The flow line ball valve leading from the well to the production equipment was in the closed position. I spoke with Mrs. Byler concerning her responsibilities and the requirements for an exempt domestic well. I found that:




According to the Exempt Domestic Well Owner Criteria, Mrs. Byler does meet the minimum requirements for an exempt domestic well owner.
