1424200851 2015-08-18 Plug / Plug Back No Violations Im on location on this date to observe plugging and abandonment activity which is commencing today. Plugging strategy will consist of top loading redi-mix cement grout with a tremmie pipe for displacement of the well fluids. The plugging contractor is Moore Well Service (Stow, OH). Plugging activities were completed on this same date, and details can be found in the associated plugging report.
1439157423 2015-07-29 Orphan No Violations I inspected this location as part of an on-going investigation to assess possible plugging strategies for a previously-undocumented well discovered during a house construction project. Currently the well is comprised of a 7-inch diameter steel well riser casing with a 3-bolt lid. There a two horizontal 1-inch female connections with are open to atmosphere. There has not been any noted gas odors or documented gas readings at the wellhead since discovery. The wellhead is noted to be approximately 30 inches below the existing grade. The landowners have expressed a desire to continue the house construction project. I have warned the landowners about the inherent risks and dangers with this project. As part of the project, they have hired Moore Well Service to assist in the assessment. At the time of this inspection, Appalachian Well Surveys is conducting a cement bond log (CBL) survey of the well. The results of the survey indicate the bottom of the well to be at 805 feet below grade. Water with several feet of oil was encountered at 70 Feet below grade. The bottom of the 7-inch casing was found to be at 100 +/- feet below grade. These data will be used to establish a plugging strategy, which is pending. At the conclusion of this I inspection, the well lid was replaced and no other activities performed.
1466777593 2016-06-10 Final Restoration No Violations I inspected this well as a evaluation of current status and for final restoration requirements. At the time of this inspection, the well has been vaulted with passive vent system which is located under the western portion of the house and not visible from the exterior. The well area has been restored as part of the house construction. Final restoration is complete. No further activity is necessary.
1765526332 2015-07-07 Orphan No Violations I inspected this well location pursuant to a request from the landowner. The presence of the well was reportedly discovered during construction of the subsurface infrastructure for a residential home. As part of the site evaluation, I invited Chris Lyons (Bay Village Fire Chief) , Steve Vogel (Bay Village Building Dept.) Also present is Ed Pavicic (General Contractor), and Jeff Moore and Keith Moore (Moore Well Service). The purpose of the meeting was to make an initial assessment of the well and discuss both ODNR and Bay Village requirements for management. At the time of this inspection, the well is comprised of a 7-inch diameter steel casing riser with a 3-bolt lid. There are two horizontal 1-inch female threaded connection points which are open to atmosphere. There is no detectable gas odors or metered readings. The well is partially enclosed within a concrete vault structure. The lid was removed for interior inspection. The interior of the 7-inch casing appears to be open. No other casings or well appurtenances can be seen. The vault structure was put in place as part of the current site activity. According Ed Pavicic, the property owners wish to continue the house construction project and are seeking guidance on plugging requirements and advice for post-plugging management. I advise that ODNR discourages the construction of homes or other structures over or near existig or former wells, but hat no regulation prohibits it under current Ohio Oil & Gas rules and regulations. We agress to confer again after the city building and zoning has reviewed the situaiton further. At the conclusion of the meeting, the well lid is replaced and no further assessment is conducted.
