| I was on site today due to an oil and brine release at the tank battery.  At the time of my arrival, the operator had started clean up work and had recovered 10 barrels of free product.  Approximately 38 barrels of brine and 2 barrels of oil was released from the 100 barrel production tank.  Operator had measured the tank on Friday, the tank had 48 inches of fluid in it.  The cause of the leak was vandalism.  The three inch valve at the bottom of the 100 barrel production tank had been removed, causing the release.  The handle for the valve had been broken off, the right hand side of the valve had the paint chipped off of it. There was a large rock in the battery that had not been there on Friday.  It is suspected that the rock was used to loosen the valve and the valve was backed off of its threads.  The battery floor had approximately 10 barrels of crude oil and brine in it.  The free product was removed with a vacuum truck.  The dike is constructed out of sandy soil.  Brine and oil seeped through the side of the dike on the south and west side. In the area immediately around the dike (within 10 feet), the top three inches of soil was saturated with brine and oil.  The brine/oil flowed 200 feet south of the battery, following an intermittent drainage swail that ended in a low spot in the woods.  The only indication of the brine was soil staining. The area has sandy soils and gravel to an unknown depth.    Advised clean-up as follows:  Remove grossly contaminated soil outside of the tank battery and replace the clean soil.  Place booms in the intermittent drain way to prevent any residual product from spreading.  Bio-remediation of the brine staining in the intermittent drainage way; mix lime and peat into soil as soil needed.  Excavate a sump at the east and west corner of the battery, to allow free  product to flow from the gravel floor of the battery.  Steam clean side of production tank.  Remove production tank from tank battery.  Steam clean floor of the bat

| On 6/11/2012, I conducted an inspection on the clean-up from a spill on 5/22/2012.  My inspection found that all contaminated soil/gravel had been removed, a new dike had been built, new gravel had been placed into the tank battery, and the oil/brine storage tank, separator, and well head were newly painted. Clean-up complete.

| My inspection today found Perry the pumper for Enervest on location checking the status of the sump, that has been constructed inside the tank battery.  Sump had 10 gallons of brine, oil and water mix in it.  Pump truck was on the way to clean sump out.  Operator had excavated and removed the tank battery dike. The operator had excavated below grade to remove all the sandy soil as I had advised. The operator back filled excavation with clay soil and built up a new dike with clay.  The size of the new dike is more than sufficient to contain 110% of the volume of the 100 barrel production tank that is in the battery.  Operator used the clean sandy soil that had been the dike as re-soiling material north of the battery as advised.  Operator is going to monitor the sump and allow any remaining brine and oil to accumulate there, to be removed.  Once all residual fluid is recovered sump will be filled in.  The operator has lined the floor of the battery with limestone, there is a sufficient stockpile of limestone in the battery to complete lining once the sump is filled  There had been an attempt by someone to steal the tank ladder that had been removed from the production tank during clean up.  Ladder was in the lease road away from the site.  The well is a plunger lift system well.  The well head is in good repair.  The heat treater separator is outside of the dike and way from the production tank. Spacing is correct at the facility.  The identification sign is posted on the production tank.  The lease road is constructed out of durable material and is stable.

By 5:00 PM the operator had placed booms, removed contaminated soil and loaded it into a lined dumpster. Sumps were constructed.  The floor of the battery and the production tank had been steam cleaned. The production tank was emptied and removed from the battery. Approximately 20 barrels of free product was recovered.  Peat was mixed into stained soil in the intermittent drainage way.
