| KEN PIISPANEN INSPECTED WELL 3/26/2009.  KEN RECEIVED A CALL FROM CARL CRAWFORD ABOUT GAS ODOR IN THE AREA OF THE VILLAGE OF PORTAGE.  CARL NOTED THAT THE ODORS STARTED IN 2007.  HE PURCHASED ADDITIONAL PROPERTY, AND HE DISCOVERED THAT THE ODOR WAS COMING FROM THE NEW PROPERTY.  MIKE COYER OF BLACK SWAMP OIL SERVICES WAS CALLED, AND THE ABANDONED WELL WAS LOCATED.  WELL WAS BURIED IN RUBBLE PILE, AND DIGGING TO EXPOSE WELL MADE MIKE SICK FROM FUMES.  RON BECHSTEIN OF PORTAGE FIRE DEPT SAID HE RECEIVES CALLS EVERY YEAR ABOUT THE ODOR, BUT SOURCE WAS UNKNOWN.  THERE WAS A STRONG H2S ODOR DURING THE INSPECTION.  GAS METER WOULD NOT OPERATE DURING TIME OF INSPECTION.  OPEN CASING HAD DIRT IN PIPE.

Village of Portage Fire Department assisted by supplying compressed air for Mike while making repair to well due to the serious health problems from H2S exposure while making repairs last time. A rubber gasket with steel clamp was placed around the weld area. This stopped the gas leak. In addition a 12 foot long PVC pipe was placed over the well and backfilled around the base. This will protect the well from vandalism and also vent gas above ground should the well start leaking again. These measures are only a temporary fix and the well needs to be properly plugged to eliminate the H2S gas problem. Members of the fire department on site confirmed that calls about gas leaking have occurred from this well. These calls not only reqired fire department response but also from the local gas company to try to find the source of the odor. The source of the gas was unknown at that time since the well was buried under a pile of rocks and debris.

1244723622 2009-06-10 Orphan No Violations Met on site with Mike Coyer to repair gas leak on well. Jeff Fry also present during inspection. Village of Portage Fire Department assisted by supplying compressed air for Mike while making repair to well due to the serious health problems from H2S exposure while making repairs last time. A rubber gasket with steel clamp was placed around the weld area. This stopped the gas leak. In addition a 12 foot long PVC pipe was placed over the well and backfilled around the base. This will protect the well from vandalism and also vent gas above ground should the well start leaking again. These measures are only a temporary fix and the well needs to be properly plugged to eliminate the H2S gas problem. Members of the fire department on site confirmed that calls about gas leaking have occurred from this well. These calls not only reqired fire department response but also from the local gas company to try to find the source of the odor. The source of the gas was unknown at that time since the well was buried under a pile of rocks and debris.

Readings of H2S from well on previous sample were 1000 ppm. Fans placed around wellhead to dispurse gas from well. Gas bled off from well and temporary containment removed. Operator working on well was working with SCBA. Anybody on site required to where H2S detector and given safety briefing before entering site. Diverter head will be attached to well when temporary device removed.
1267546866 2010-03-02 Plug / Plug Back No Violations Cement pumped into well on 3/1/10 set up at depth of 875 feet. There was approximately 75 feet of water on top of cement in well . Operator gravity poured 7 yards of 9 sack grout into well. Cement rose to surface of well with no fluids to surface. Bradner Oil, Marathon Oil and Total Safety on site for plugging.
