Oil and Gas 2009-04-08 KEN PIISPANEN INSPECTED WELL 3/26/2009. KEN RECEIVED A CALL FROM CARL CRAWFORD ABOUT GAS ODOR IN THE AREA OF THE VILLAGE OF PORTAGE. CARL NOTED THAT THE ODORS STARTED IN 2007. HE PURCHASED ADDITIONAL PROPERTY, AND HE DISCOVERED THAT THE ODOR WAS COMING FROM THE NEW PROPERTY. MIKE COYER OF BLACK SWAMP OIL SERVICES WAS CALLED, AND THE ABANDONED WELL WAS LOCATED. WELL WAS BURIED IN RUBBLE PILE, AND DIGGING TO EXPOSE WELL MADE MIKE SICK FROM FUMES. RON BECHSTEIN OF PORTAGE FIRE DEPT SAID HE RECEIVES CALLS EVERY YEAR ABOUT THE ODOR, BUT SOURCE WAS UNKNOWN. THERE WAS A STRONG H2S ODOR DURING THE INSPECTION. GAS METER WOULD NOT OPERATE DURING TIME OF INSPECTION. OPEN CASING HAD DIRT IN PIPE.

0486517203 2009-05-19 Orphan No Violations Area around well has been excavated to top of rock to expose well casing. Mike Coyer was on site and has welded control head on well. Valve installed on well to allow release of pressure before plugging. Gas pressure built up in well after being closed for a few minutes and flame burned 6-10 feet high.
0502871959 2010-03-01 Plug / Plug Back No Violations Diverter succcesfully attached to well after some problems. Well was cleaned out to depth of 1133 feet. Several bridges were drilled through to reach total depth. Fluids in well suppressed gas so H2S not a problem. Operator pumped 126 sacks Class A cement into well. Fluids displaced to tank.. Cement will be allowed to set up over night and tag depth in morning. Bradner Oil, Marathon Oil and Total Safety on site.
1244558938 2009-06-08 Orphan No Violations Checked well. Gas is leaking from area where welding was done to attach well head control. Spoke to Mike Coyer by phone. He said he has tried to repair small leaks by welding and use of JB Weld material. The old casing is deteriorated and difficult to repair. Recommends clamping rubber sleeve around casing to help contain gas.
1244723622 2009-06-10 Orphan No Violations Met on site with Mike Coyer to repair gas leak on well. Jeff Fry also present during inspection. Village of Portage Fire Department assisted by supplying compressed air for Mike while making repair to well due to the serious health problems from H2S exposure while making repairs last time. A rubber gasket with steel clamp was placed around the weld area. This stopped the gas leak. In addition a 12 foot long PVC pipe was placed over the well and backfilled around the base. This will protect the well from vandalism and also vent gas above ground should the well start leaking again. These measures are only a temporary fix and the well needs to be properly plugged to eliminate the H2S gas problem. Members of the fire department on site confirmed that calls about gas leaking have occurred from this well. These calls not only reqired fire department response but also from the local gas company to try to find the source of the odor. The source of the gas was unknown at that time since the well was buried under a pile of rocks and debris.
1247516242 2009-07-13 Orphan No Violations Checked repairs made on well to stop H2S gas release. Gas odor is obvious standing down wind of riser pipe. Additional gas leaks are suspected from deteriorated casing on well. Will contact Jeff Fry and Mike McCormac about condition at well.
1247679612 2009-07-14 Orphan No Violations Checked well with Jeff Fry. There was some odor of gas but not as evident as on the previous day.
1267462787 2010-02-24 Orphan No Violations Bradner Oil, Marathon Oil and Total Safety on site to begin working on well . Total Safety monitoring for H2S gas around wellhead. Readings of H2S from well on previous sample were 1000 ppm. Fans placed around wellhead to dispurse gas from well. Gas bled off from well and temporary containment removed. Operator working on well was working with SCBA. Anybody on site required to where H2S detector and given safety briefing before entering site. Diverter head will be attached to well when temporary device removed.
1267546866 2010-03-02 Plug / Plug Back No Violations Cement pumped into well on 3/1/10 set up at depth of 875 feet. There was approximately 75 feet of water on top of cement in well . Operator gravity poured 7 yards of 9 sack grout into well. Cement rose to surface of well with no fluids to surface. Bradner Oil, Marathon Oil and Total Safety on site for plugging.
1268835061 2010-03-03 Orphan No Violations Bradner Oil and Marathon employee Mondo on site removing equipment from site. Well cut off 4 feet below ground and ID plate attached. Cement to top of well casing.
1273753495 2010-05-10 Final Restoration No Violations Checked site for final reclamation. Area around well has not yet been final graded or seeded. Called Bob Harpster. He is aware site has not been completed. Conditions have been too wet to roto till area for seeding. Area will be roto tilled and seeded as soon as conditions allow.
1282748520 2010-08-25 Final Restoration No Violations Final restoration done.
1429951240 2010-02-25 Orphan No Violations Bradner Oil, Marathon Oil and Total Safety on site to continue to prepare well for plugging. Rotary drill set up over well. Bridge at top of well removed and well was flooded with brine to suppress gas. Gas monitoring done at wellhead before attaching diverter. Same safety precautions as on previous day except no SCBA required at wellhead.
1955593454 2010-03-04 Orphan No Violations Equipment removed from site and area rough graded. Ground conditions muddy so final grading and seeding will be done when conditions improve.
