| The purpose of my inspection was to conduct a status check on well #2-2661.  A review of RBDMS indicated the owner reported under the minimum reported production for 2015.  Lease access was through a lease road shared with well # 2-3172-01.  A culvert drain was located at the entrance of the unshared portion of the lease road.  The access road was partially overgrown and a fallen tree blocked the road.    The well location was stable with well-established vegetation.  No identification was present at the well head.  The 8.625 well head was above grade.  The annular side ports contained a plug in one side and a closed valve in the other side.  The 4.5 production casing was equipped with a plunger lift assembly.  Production valves were in the closed position.  No audible visual leaks were observed.  An earthen dike that was stable and free of fluid was constructed around the production equipment.  Equipment within the dike consisted of one (1) horizontal separator and one (1) 100 bbl production tank.  The doors to the separator were open.  The meter house contained an expired production chart.  The identification sign on the production tank was peeling and information was faded.  The tank body remained in good repair.  No holes or leaks were observed. The load line and drain line valve were closed and did not contain plugs.  Energy Resources of America shall post correct and legible identification at the well head and tank battery.  Tank valves within the battery shall be secured with bull plugs.    Vegetation at the well head access road and tank battery shall be maintained consistent with reasonably prudent operations.  Compliance activities shall be completed within thirty (30) days of receipt of this notice.

At the time of inspection correct and legible identification was posted at the tank battery. Bull plugs were installed in the tank valves. No further action was necessary.
1402930061 2014-05-08 Production Wells No Violations I performed a status check on the above location. The tank battery is equipped with one 100 barrel tank and one horizontal separator. The well is equipped with 1.5 tubing. No noted violations at this time. ID signs are posted.
1497371586 2017-06-12 Production Wells Violations Noted The purpose of my inspection was to conduct a status check on well #2-2661. A review of RBDMS indicated the owner reported under the minimum reported production for 2015. Lease access was through a lease road shared with well # 2-3172-01. A culvert drain was located at the entrance of the unshared portion of the lease road. The access road was partially overgrown and a fallen tree blocked the road. The well location was stable with well-established vegetation. No identification was present at the well head. The 8.625 well head was above grade. The annular side ports contained a plug in one side and a closed valve in the other side. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the closed position. No audible visual leaks were observed. An earthen dike that was stable and free of fluid was constructed around the production equipment. Equipment within the dike consisted of one (1) horizontal separator and one (1) 100 bbl production tank. The doors to the separator were open. The meter house contained an expired production chart. The identification sign on the production tank was peeling and information was faded. The tank body remained in good repair. No holes or leaks were observed. The load line and drain line valve were closed and did not contain plugs. Energy Resources of America shall post correct and legible identification at the well head and tank battery. Tank valves within the battery shall be secured with bull plugs. Vegetation at the well head access road and tank battery shall be maintained consistent with reasonably prudent operations. Compliance activities shall be completed within thirty (30) days of receipt of this notice.
