| I inspected this well and found that the operator, Landmark 4 LLC, had recently removed the pump jack unit from the wellhead. The pump jack, electric motor and all electrical lines have been removed from the site. The electric meter at the road for the pump jack motor has been disconnected at the weatherhead on the pole. The wellhead itself is still in place and there is a gate valve on the wellhead that has been shut in. The tubing and rods are still in place and a rod section is currently sticking out of the well. The area around the wellhead has been rough graded and a trackhoe belonging to Landmark 4 LLC was on site parked next to the wellhead. I could not determine any pressures on the well as there were no gauges installed. I noticed several sections of old steel flowlines that had been removed from the ground and strewn about the area around the well. The tank battery for this well has also been removed and all production equipment from the battery are gone. The area of the former tank battery has been rough graded. I noticed several sections of casing a tubing laying on the ground near the graded area. I also noticed several sections of steel flow lines that had been broken and ripped from the ground and are now sticking up out of the ground. This former tank battery was also handling production for the E P Fuller #1 well. Landmark 4 LLC has a current plug permit on file with an issue date of 09/21/2012. I attempted to contact Steve Swain of Landmark 4 LLC and left message with his office to contact me. The crew tripped out 2225 ft. of 5.5 in. casing, which was a total of 70 joints of 5.5 in. casing and stacked it on a trailer. I observed several large holes in the 5.5 in. casing at approx. the 2100 ft. depth level. The service rig crew ran 1.5 in. tubing to a depth of 2100 ft. for placement of the cement plug. Petroset arrived on site and rigged up their cementing equipment. Petroset began pumping and broke circulation and then pumped 80 sks. of Class A cement with 2% gel and placed the cement plug from 2100 ft. to 1800 ft. The cement plug was displaced with 4.5 bbls. of fresh water. The service rig crew then pulled the 1.5 in. tubing to a depth of 550 ft. Petroset then hooked up again to the 1.5 in. tubing and pumped fresh water to break circulation. They then pumped 55 sks. of Class A cement with 2% gel and placed the plug from 550 ft. to 350 ft. The cement plug was displaced with 1 bbl. of fresh water. The service rig crew then pulled the 1.5 in. tubing up to 300 ft. Petroset then hooked back up to the 1.5 in. tubing and began pumping fresh water and broke circulation. They then pumped 100 sks. of Class A cement with 2% gel and placed the cement plug from 300 ft. to the surface and I witnessed the cement return to the surface. The service rig crew tripped out the remainder of 1.5 in. tubing and stacked it on a trailer. It was determined not to pull the 8 in. casing due to it being full of holes and fearing that it may rip apart during the pulling process. The cement did circulate up the 8 in. annulus during the placement of the surface plug. After approx. ten minutes, the cement settled down about ten feet into the well so Petroset pumped an additional amount through their hose in order to top of the surface plug. | ||

I inspected a 30-foot radius from the given coordinates and found no evidence of Pollution and/or contamination or any evidence of any ruts or gravel left by the plugging operation. I concur with the passing of the final restoration for this well. | ||

The crew then pulled approx. 2900 ft. of 2.875 in. tubing and the latch packer and stacked them on a trailer. A total of 96 joints of 2.875 in. tubing was removed from the well. The service rig crew then ran 1.5 in. tubing into the well to a depth of 3256 ft. in order to place the cement plug. Petroset arrived on location and rigged up their cementing equipment to the 1.5 in. tubing. Petroset pumped fresh water and broke circulation and pumped 25 sks. of Class A cement with 2% gel and placed the bottom plug from 3256 ft. to 3050 ft. The cement plug was displaced with 8 bbls. of fresh water. The service rig crew then tripped out 3256 ft. of the 1.5 in tubing and stacked it on a pipe rack. A total of 96 joints of 1.5 in. tubing was removed from the well. The service rig crew then rigged up a 5.5 in. ripping tool and ripped the 5.5 in. casing at 2225 ft. Petroset then hooked up to the 5.5 in casing and began pumping and circulated oil and brine out of the annulus until the water was clear in color. Then plugging operations were finished for the day. | |

The pump jack, electric motor and all electrical lines have been removed from the site. The electric meter at the road for the pump jack motor has been disconnected at the weatherhead on the pole. The wellhead itself is still in place and there is a gate valve on the wellhead that has been shut in. The tubing and rods are still in place and a rod section is currently sticking out of the well. The area around the wellhead has been rough graded and a trackhoe belonging to Landmark 4 LLC was on site parked next to the wellhead. I could not determine any pressures on the well as there were no gauges installed. I noticed several sections of old steel flowlines that had been removed from the ground and strewn about the area around the well. The tank battery for this well has also been removed and all production equipment from the battery are gone. The area of the former tank battery has been rough graded. I noticed several sections of casing a tubing laying on the ground near the graded area. I also noticed several sections of steel flow lines that had been broken and ripped from the ground and are now sticking up out of the ground. This former tank battery was also handling production for the E P Fuller #1 well. Landmark 4 LLC has a current plug permit on file with an issue date of 09/21/2012. I attempted to contact Steve Swain of Landmark 4 LLC and left message with his office to contact me. I received no call back. |

The crew tripped out 2225 ft. of 5.5 in. casing, which was a total of 70 joints of 5.5 in. casing and stacked it on a trailer. I observed several large holes in the 5.5 in. casing at approx. the 2100 ft. depth level. The service rig crew ran 1.5 in. tubing to a depth of 2100 ft. for placement of the cement plug. Petroset arrived on site and rigged up their cementing equipment. Petroset began pumping and broke circulation and then pumped 80 sks. of Class A cement with 2% gel and placed the cement plug from 2100 ft. to 1800 ft. The cement plug was displaced with 4.5 bbls. of fresh water. The service rig crew then pulled the 1.5 in. tubing to a depth of 550 ft. Petroset then hooked up again to the 1.5 in. tubing and pumped fresh water to break circulation. They then pumped 55 sks. of Class A cement with 2% gel and placed the plug from 550 ft. to 350 ft. The cement plug was displaced with 1 bbl. of fresh water. The service rig crew then pulled the 1.5 in. tubing up to 300 ft. Petroset then hooked back up to the 1.5 in. tubing and began pumping fresh water and broke circulation. They then pumped 100 sks. of Class A cement with 2% gel and placed the cement plug from 300 ft. to the surface and I witnessed the cement return to the surface. The service rig crew tripped out the remainder of 1.5 in. tubing and stacked it on a trailer. It was determined not to pull the 8 in. casing due to it being full of holes and fearing that it may rip apart during the pulling process. The cement did circulate up the 8 in. annulus during the placement of the surface plug. After approx. ten minutes, the cement settled down about ten feet into the well so Petroset pumped an additional amount through their hose in order to top of the surface plug. |

Met at the access road off Plum Creek Parkway at the (Lat/Long 41.21583, -81.80739). The company personnel (Steve Swain and Jim Schmidt) showed me the approximate location of the well which was real close to the Latitude and Longitude (41.21592, -81.80621) on file in well data base of the Division of Oil and Gas Resources Management. I inspected a 30-foot radius from the given coordinates and found no evidence of Pollution and/or contamination or any evidence of any ruts or gravel left by the plugging operation. I concur with the passing of the final restoration for this well.
Well History: This well was plugged on 11-20-2012 was witnessed by Division of Oil and Gas Resources Management Field Inspector Brian Logue also a final restoration inspection was conducted by DOGRM Field Inspector Amada Trimble on 08-09-2013. |

The crew then lined the pit with a 12 mil. plastic liner. The service rig crew from Thompson Well Service was on site and was in the process of releasing the 2.875 in. latch packer. The crew then pulled approx. 2900 ft. of 2.875 in. tubing and the latch packer and stacked them on a trailer. A total of 96 joints of 2.875 in. tubing was removed from the well. The service rig crew then ran 1.5 in. tubing into the well to a depth of 3256 ft. in order to place the cement plug. Petroset arrived on location and rigged up their cementing equipment to the 1.5 in. tubing. Petroset pumped fresh water and broke circulation and pumped 25 sks. of Class A cement with 2% gel and placed the bottom plug from 3256 ft. to 3050 ft. The cement plug was displaced with 8 bbls. of fresh water. The service rig crew then tripped out 3256 ft. of the 1.5 in tubing and stacked it on a pipe rack. A total of 96 joints of 1.5 in. tubing was removed from the well. The service rig crew then rigged up a 5.5 in. ripping tool and ripped the 5.5 in. casing at 2225 ft. Petroset then hooked up to the 5.5 in casing and began pumping and circulated oil and brine out of the annulus until the water was clear in color. Then plugging operations were finished for the day. |
