| Inspect well. Gas well. 4 inch long string heavily rusted at swage, reduced to 2 inch production. 7 inch surface casing. Well shut and valve has failed. WELL HEAD IS LEAKING GAS WITH RESIDUAL OIL. AND, THIS WELL APPEARS TO BE CHARGING THE PRODUCTION LINE AND IS LEAKING AT THE CAWRSE #2  (PERMIT 221) WELL. STRONG HYDROGEN SULFIDE ODORS. Measured with 4 gas meter; exceeded LEL levels and H2S in excess of 250 ppm escaping from well head.

| The current landowner has submitted documentation to produce the well for domestic purposes.  The well head pressure was 29 psi on gauge. Tested gas quality with Sperian PhD6 four gas meter three individual times. The first set of readings maxed the Methane and Lower Explosive Limit (LEL), Carbon monoxide (CO) reading was not noted, Hydrogen Sulfide (H2S) has a reading of 2 parts per million (PPM).  The meter was allowed to clear and was restarted as a result of the max readings. The second set of readings indicated 0.85 % Methane, 35 % LEL, 9 ppm CO, and Zero hydrogen sulfide.  The third set of reading were collected; Methane was 4.0%, LEL reached 100% then maxed out the sensor, the hydrogen sulfide was zero ppm. The gas reading for this well were collected in July after the bottom plug was set and the 4.5 inch casing was ripped and removed. These results are consistent with the previous results. Otherwise the offer for the well to the landowner would not have been made. A seven inch to two inch swage, ball valve, and regulator were installed on the well by the plugging contractor. A gas line to the house has been laid and buried. The landowner is waiting for final transfer for use. TSB.

0469405169 2004-07-28 Production Wells No Violations Inspect gas well. Well shut in, the valve is cracked open slightly. Pipe fitting are rusted and deteriorated. Poor condition. H2S odor, no readings.
0946740498 2003-04-04 Production Wells No Violations Inspect Well. Gas Well. 8 pipe and slips to 4 pipe reduced to 2 pipe, Tee, and valve. The valve is partially open. Gas is seeping from well, and is gurgling.
1034688912 2004-06-17 Production Wells No Violations Inspect tanks, update tank location. 1 - 100 bbl common tank with permits 174, 219, and 221.
1125631549 2004-11-09 Production Wells No Violations Inspect well. Well shut in, valve is completely closed. Well head is rusted. Strong H2S odor at well head. No ID posted at well head. ID is posted at common tank for wells, permits number 174, 219, and 221. The 100 bbl tank is in deteriorated state.
1138362861 2008-04-23 Production Wells No Violations Inspect well.
1239640718 2012-07-18 Plug / Plug Back No Violations Petrocement Services set bottom plug on well with 50/50 poz cement.
1308320713 2011-05-09 Orphan No Violations Inspect well. Well with 4.5 inch casing, well head, swage to 2 inch, gas well, no rods, no pump jack.
1359063022 2013-01-24 Production Wells No Violations The current landowner has submitted documentation to produce the well for domestic purposes. TSB.
