| Orphan well field work: The purpose of this site visit was to dig up well casing and attempt to run a weighted line into well to check for obstructions on the tubing-casing annulus. Upon digging down and exposing the casing, the well was observed to be steadily leaking gas and brine from the tubing-casing annulus. After using a steel soil probe to probe into the tubing-casing annulus, the flow rate of gas and brine from the 7 casing increased and the hole began filling with brine. A slight skim of crude oil was present on top of the brine, however no more oil was being produced. As a precaution, a call was put in to the DOGRM One-Call spill hotline to notify them of the situation in case it worsened. A 20 long by 1 wide by 1 deep trench was dug on the down gradient side of the well to contain any fluids that may flow out of the depression. At the time of departure, the flow rate of gas and brine from the wellbore was slowing down and no more crude oil was observed. Photos were taken of the well location.

| Orphan well investigation: Upon inspection, it is observed that no well is present at the surface at or near coordinates supplied by a 2005 Division inspection. There is a bare area in an agricultural field very close to the supplied coordinates (~30). The landowner, Mr. Elmer Peachey, states that he cut the well off below the surface with a hacksaw nearly a year ago and buried it under his field so he could farm over it. Mr. Peachey also states that the well had been idle ever since his family purchased the farm 20+ years ago, but was connected to a greenhouse on the farm prior to their ownership. Mr. Peachey also states that the well still released gas to the atmosphere from time to time before he cut it off and buried it. The well releasing gas is likely the cause of the kill area over the well location now. No trees or vegetation will need to be cleared to access the well. Steel plates will be needed to cross an existing earthen bridge and culvert to the well location.  Mr. Peachey has been provided with a copy of the Plugging Application and Right of Entry Form and states he will fill it out and return it to the Division. The Site Evaluation Matrix Score for this well is 203. Pictures were taken.

| Orphan Well Status Inspection: Upon inspection, it is observed that the well is buried under an agricultural field and is leaking gas and brine to the surface. The vegetation kill area is still present around the well location and has grown to over 200 square feet in size. This well will be revisited in the coming weeks and located with a metal detector and dug up to see which casing strings are still present. Photos were taken of the well location and access point. This well is included in the Jackson #2 Orphan Well Plugging Project which is scheduled to be bid out in July 2018.

| Orphan well status inspection, accompanied by DOGRM staff members Rob Lowe, Lucas Smith, Trisha Cartmell, and Nate Lane: The purpose of this inspection is to familiarize DOGRM technical sections with this well location so they can perform their individual assessments for the Jackson #2 Plugging Project. During this inspection, Survey Technician Lucas Smith used a metal detector to locate the well casing and the well was dug up with a shovel to expose the casings. Upon exposing the well, it is observed that the well consists of 8.63 diameter casing, 7 diameter casing, and 2.38 diameter tubing. The 5.5 diameter casing mentioned in the RBDMS records does not appear to be present at the surface and may have been pulled. The 2.38 diameter tubing is likely set on a packer above the Ohio Shale. There is gas and brine leaking from the 7 diameter casing. The 2.38 diameter tubing looks to have a cement-like material plugging it off where it was cut with a hacksaw. Photos were taken of these findings.

The landowner, Mr. Elmer Peachey, states that he cut the well off below the surface with a hacksaw nearly a year ago and buried it under his field so he could farm over it. Mr. Peachey also states that the well had been idle ever since his family purchased the farm 20+ years ago, but was connected to a greenhouse on the farm prior to their ownership. Mr. Peachey also states that the well still released gas to the atmosphere from time to time before he cut it off and buried it. The well releasing gas is likely the cause of the kill area over the well location now. No trees or vegetation will need to be cleared to access the well. Steel plates will be needed to cross an existing earthen bridge and culvert to the well location. Mr. Peachey has been provided with a copy of the Plugging Application and Right of Entry Form and states he will fill it out and return it to the Division. The Site Evaluation Matrix Score for this well is 203. Pictures were taken.
2070070573 2018-04-06 Orphan No Violations Orphan Well Status Inspection: Upon inspection, it is observed that the well is buried under an agricultural field and is leaking gas and brine to the surface. The vegetation kill area is still present around the well location and has grown to over 200 square feet in size. This well will be revisited in the coming weeks and located with a metal detector and dug up to see which casing strings are still present. Photos were taken of the well location and access point. This well is included in the Jackson #2 Orphan Well Plugging Project which is scheduled to be bid out in July 2018.
2076281295 2018-05-08 Orphan No Violations Orphan well status inspection, accompanied by DOGRM staff members Rob Lowe, Lucas Smith, Trisha Cartmell, and Nate Lane: The purpose of this inspection is to familiarize DOGRM technical sections with this well location so they can perform their individual assessments for the Jackson #2 Plugging Project. During this inspection, Survey Technician Lucas Smith used a metal detector to locate the well casing and the well was dug up with a shovel to expose the casings. Upon exposing the well, it is observed that the well consists of 8.63 diameter casing, 7 diameter casing, and 2.38 diameter tubing. The 5.5 diameter casing mentioned in the RBDMS records does not appear to be present at the surface and may have been pulled. The 2.38 diameter tubing is likely set on a packer above the Ohio Shale. There is gas and brine leaking from the 7 diameter casing. The 2.38 diameter tubing looks to have a cement-like material plugging it off where it was cut with a hacksaw. Photos were taken of these findings.
