|  Status check inspection found (3) wells total on this well pad with all 3 wells in production. All 3 wells are producing through a production tree through 3 manual gate valves and one emergency shut down valve with an electric choke present in line. The said well does have 2 diameter riser pipes present attached to the well head A&B sections outlet ports. The riser pipes are equipped with valves and gauges (both electronic transducers and manual gauges) the purpose of the gauges is to monitor annular pressure. The A section well head pressure is 325 (PSI) pounds per square inch, the B section pressure is 1,200 psi. The 5.5 diameter production casing pressure is 4,200 psi and the flowline pressure down stream from the choke is 1,600 psi. There is an emergency shut down control point at each of the 3 well head along with each critical point on the well pad. On site production equipment consists of (3) gas processing units along with a tank battery that contains (4) steel tanks capable of holding 400 barrels of fluid each along with a steel 50 barrel tank, all situated inside a polymer lined steel containment dike area. There is a third party (EQT) midstream pipeline station adjacent to the right front corner of the well pad. The lease road and well pad surface is stone construction and in good repair. The well pad per miter is protected by an earthen berm. The Preliminary Restoration is due however there is snow on the ground during my inspection.

1501520618 2017-07-31 Drill / Deepen / Reopen No Violations During my inspection I witnessed Sidewinder 103 reach a total depth of 19,208 feet. Company man Derrick intends to run 19,183 feet of 5.5 inch diameter casing in the next few days. Inspection revealed that containment appears to be functioning correctly and pad drains are shut in and accumulating water.
1546487242 2018-02-14 Completion Testing No Violations This well is equipped with 5.5-inch diameter production casing. The well is currently shut in as no completion operations have commenced. There are mats currently stacked out on the side of the location. There is midstream equipment that has been installed and is awaiting completion testing following which, production operation will be tied in.
1547572847 2019-01-15 Production Wells No Violations Status check inspection found (3) wells total on this well pad with all 3 wells in production.
