| On the date of my inspection I found the tank battery to include 3 properly identified 100 barrel tanks, a concrete brine vault, a vertical separator, and a production meter box with a current chart, a drip, and a domestic diaphragm meter, within an earthen dike.  Behind the northern most tank there is an area approximately 3 feet by 10 feet where the ground is saturated with brine (Quantab >10,000 ppm).  A pool of brine approximately 4 inches deep and 2 feet long is located below a valve at the bottom of the tank.  The well is equipped with a pumping unit, rods, 2.375 inch tubing, and 4.5 inch casing.  The tubing valve is in the open position.  Adjacent to the well head there is an area approximately 3 feet by 5 feet where oil and brine have accumulated.  Production records end in 2013. Pictures were taken.  I spoke with Jim Posey with Samurai Corp, will meet to discuss clean up on 8/11/16 at 10 AM.

| My inspection found a well maintained lease road leading up to three 210 barrel production tanks and a horizontal separator contained in a dike. Located outside the dike was a out of use brine vault from the original owner of the well. The well has a pump jack located on it. There was a small cut in the dike that could containment of a spill. I contacted Samurai representative Michael Stone and he stated that there was an order for materials to replace the dyke in process. There were no violations at this time.

The tank battery is not properly contained in a SPCC dike. A brine vault in use is outside of the tank battery and is also not contained in an SPCC dike. There is a small 3x5 spot on the ground were a very small amount of oil and some brine has been spilled. The sun has dried the brine and salt crystals are visible evident. The well needs proper signage, SPCC dikes and the small area of contamination needs to be cleaned up.

| Today I am following up on a small oil leak around the wellhead noted on 10/19/17.  The accumulated oil has been removed and fresh gravel has been placed around the wellhead.  Production equipment includes 3 properly identified 100 barrel tanks, a concrete brine vault, a vertical separator, and a production meter box with an orifice plate and a current chart, 2 drips, and a domestic diaphragm meter, within an earthen dike.  The wellhead is equipped with a pumping unit, rods, 2.375 inch tubing, and 4.5 inch casing.  A 2.375 inch production line ties the tubing and casing together then runs to the production tanks.  Both the tubing and casing valves are open.  Gauges on the both lines read approximately 45 pounds per square inch.  Production records are current.  Since my last inspection, the rods on the rack near the wellhead have been cut into smaller pieces and are on the ground near the wellhead.  The pumper for Samurai has been cutting them down for disposal.

0026564032 2016-08-10 Production Wells No Violations On the date of my inspection I found the tank battery to include 3 properly identified 100 barrel tanks, a concrete brine vault, a vertical separator, and a production meter box with a current chart, a drip, and a domestic diaphragm meter, within an earthen dike. Behind the northern most tank there is an area approximately 3 feet by 10 feet where the ground is saturated with brine (Quantab >10,000 ppm). A pool of brine approximately 4 inches deep and 2 feet long is located below a valve at the bottom of the tank. The well is equipped with a pumping unit, rods, 2.375 inch tubing, and 4.5 inch casing. The tubing valve is in the open position. Adjacent to the well head there is an area approximately 3 feet by 5 feet where oil and brine have accumulated. Production records end in 2013. Pictures were taken. I spoke with Jim Posey with Samurai Corp, will meet to discuss clean up on 8/11/16 at 10 AM.
0089262543 2012-10-23 Production Wells No Violations My inspection found a well maintained lease road leading up to three 210 barrel production tanks and a horizontal separator contained in a dike. Located outside the dike was a out of use brine vault from the original owner of the well. The well has a pump jack located on it. There was a small cut in the dike that could containment of a spill. I contacted Samurai representative Michael Stone and he stated that there was an order for materials to replace the dyke in process. There were no violations at this time.
0160546385 2013-04-24 Production Wells Violations Noted I was conducting a status check of this well. The pumpjack is located down a lease road in a field behind a house. The tank battery contains 3 production tanks and a vertical separator. The tank battery is not properly contained in a SPCC dike. A brine vault in use is outside of the tank battery and is also not contained in an SPCC dike. There is a small 3x5 spot on the ground were a very small amount of oil and some brine has been spilled. The sun has dried the brine and salt crystals are visible evident. The well needs proper signage, SPCC dikes and the small area of contamination needs to be cleaned up.

A gauge on the production line from the tubing reads 70 pounds per square inch and a gauge on the production line from the casing reads 110 pounds per square inch. Production records are current. Near the wellhead there is a metal rack with old rods on it.

He had already changed the leaking valve. He indicated that he would remove the remaining stained material and replace with clean stone and soil within the tank dike. He also indicated that he would clean up the area around the pumping unit.

There appears to be a leak around the stuffing box when the pumping unit is operating. The well tender was notified of the issue will be working repairing and cleaning it up today. Pictures were taken.
0831294759 2013-05-28 Production Wells No Violations I was onsite to check the status of this well in bringing it into compliance. An extension to the compliance notice has been granted as part of the well issues have been addressed. A dike around the salt brine tank has been installed. The unused production tank in the batter is going to be removed and the dike is going to be shortened and built up to proper height. The Samurai representative stated that Northeast Energy will be doing this work next week. I will follow up when this work is completed.
1035091018 2016-08-29 Production Wells No Violations I am on site today to follow up on my findings from a previous inspection on 8/11/16. The operator has placed peatsorb around the well head and will be removing the contamination soon. Contamination within the tank berm behind the tank has been removed and new gravel has been placed on that area as well. The leaking transfer line on the tank has been replaced. Pictures were taken.

1176953800 2013-05-16 Administrative Inspection No Violations I was in contact will the pumper for Samurai regarding the 2 compliance notices that were issued for some issues at their wells. This well had a brine vault outside the SPCC dike that needed cleaned up and a dike built surrounding it. A contractor will be onsite today to see how to build and fix the current SPCC dikes. Signage for the wells has also been ordered and will be installed next week. I will conduct a status check to determine if the wells are brought into compliance.

1236605070 2013-07-16 Production Wells No Violations I was onsite to check the work being done on the tank battery for this well regarding installing a new dike, signage and the cleanup of some oil and brine around the brine vault. All the issues have been addressed and the well is now in compliance. A new dike has been built around both the dike and the tank battery. New signage has been installed on the production tanks and the brine and oil has been cleaned up. There are no violations at this time.
1371578584 2013-06-18 Production Wells No Violations I was onsite to check the status of installing a new dike around the tank battery and shortening of the tank battery. The operator has no completed the required work at this time but they do have time in the compliance agreement to finish the work and they have been in contact with me regarding it needed work. I will check back when the work is completed.
1474160557 2017-12-19 Production Wells No Violations Today I am following up on a small oil leak around the wellhead noted on 10/19/17. The accumulated oil has been removed and fresh gravel has been placed around the wellhead. Production equipment includes 3 properly identified 100 barrel tanks, a concrete brine vault, a vertical separator, and a production meter box with an orifice plate and a current chart, 2 drips, and a domestic diaphragm meter, within an earthen dike. The wellhead is equipped with a pumping unit, rods, 2.375 inch tubing, and 4.5 inch casing. A 2.375 inch production line ties the tubing and casing together then runs to the production tanks. Both the tubing and casing valves are open. Gauges on the both lines read approximately 45 pounds per square inch. Production records are current. Since my last inspection, the rods on the rack near the wellhead have been cut into smaller pieces and are on the ground near the wellhead. The pumper for Samurai has been cutting them down for disposal.

A gauge on the production line from the tubing reads 50 pounds per square inch and a gauge on the production line from the casing reads 20 pounds per square inch. Production records are current. Near the wellhead there is a metal rack with old rods on it. I notified Jim Posey with Samurai Corporation of the issue. He indicated that he would clean it up.
