| On the day of my inspection, I witnessed Chipco LLC break circulation with fresh water then displace sixty sacks of Class A cement plug (200') at the bottom of the 8.625 casing which was through the Berea formation then pull twenty nine joints of 4.5 production casing and check the fluid level, then the remaining five joints of the 4.5 casing was pulled and sixty sack (200') Class A cement plug was displaced to the surface, I instructed the operator to cut the casing three foot below ground level, then place a cap on the casing with the date and I.D. then restore the disturbed area.
