The well was equipped with 8.625 inch diameter casing; 4.5 inch diameter casing, 1.5 inch diameter tubing and plunger lift system with the flowline only connected to the tubing. Valves were closed on the casing and the in the closed position on the tubing. Utilizing an ODNR Tango Hydrogen Sulfide (H2S) detector revealed no H2S detected. There is one - 100 barrel tank and separator in diked tank battery area. No violations noted at this time.

1391539894 2014-02-03 Production Wells No Violations On the day of my inspection, I found the well capable of production, I reviewed The Division of Oil and Gas production records and found them current for the year of 2011 and 2012, the well is equipped with eight and five eights surface casing / four and a half inch production casing / inch and a half tubing, the valves were in the open position, I meet with the well tender Terry Miller for Enervest to look at a gas line rupture on there sales line that shut off the house gas to the landowners home, Mr. Miller made the repairs to restore the land owners house gas, Mr. Miller commented to me the repairs will be made to the sales line on 01/06/2014. There is a separator / drip / chart and (1) one hundred barrel production tank setting inside a containment dike, the I.D. is posted on the tank.
2135724443 2017-08-21 Production Wells No Violations On the day of my inspection following up from a reported gas leak on Friday 8/17/17, I met with landowner Ron Fuller who accompanied me to the production equipment which included a 100 barrel tank, a horizontal separator, a motor valve, a 2 inch salesmeter with a .75 inch orifice and chart recorder containing a current chart all within an earthen dike. The well was equipped with 8.625 inch diameter casing; 4.5 inch diameter casing, 1.5 inch diameter tubing and plunger lift system with the flowline only connected to the tubing. Valves were closed on the casing and the in the closed position on the tubing. Utilizing an ODNR Tango Hydrogen Sulfide (H2S) detector revealed no H2S detected. Mr. Fuller then escorted me to where the gas leak was noted on Friday and gas was detected at Global Positioning System coordinates 40.126731 -81.276037. I placed call to Enervest and requested the leak be addressed. Upon leaving I was approached by adjoining property owner Matt Crislip and a conversation was held pertaining to his well the 34059231290000 Crislip 3.
