| On the day of my inspection I witnessed the mine string cement job. Falcon 26 drilled a 24 inch diameter open hole to a depth of 277 feet. Previous casing set and cemented was 30 inch diameter conductor casing to a depth of 100 feet. Wyoming Casing ran 275 feet of 18.625 inch diameter casing, j-55, 87.5 lbs/foot, with a 3 foot stick up. Casing was not centralized. Shoe track is 23 feet long. Cement Baskets were set at 175 feet. #8A coal encountered from 194-199 feet and #8 coal encountered from 220-227. No loss of returns while drilling. CJ/O-tex was onsite with 515 sacks of class A cement. Additives include 3% CaCl and 0.2% X-air.  Cement weighed with mudscale at 15.8 ppg and yield of 1.17. 50% excess ran in open hole. Cement displaced with 79 barrels of fresh water. 35 barrels of gel pumped ahead of cement. No cement returned to surface. Cement tagged around 50 feet. Ran 1 inch diameter tubing to approximately 50 feet. Pumped 40 barrels of cement and cement came within 5 feet of cellar bottom and fell back to 100 feet. Pulled tubing from hole and order more cement. I left job and Inspector Corder will cover the second top out this evening.

0009672082 2018-04-25 Casing String Witnessed No Violations Pad address is 640 TOWNSHIP ROAD 107A DILLONVALE 43917. On the day of my inspection I witnessed Precision Rathole had augured a 36 inch diameter hole to a depth of 100 feet. 100 feet of 30 inch diameter conductor casing was run. CJ/Otex was onsite with 310 sacks of class A cement. Additives include 2% CaCl and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% percent excess ran. . Cement was displaced with 75 barrels of fresh water. 11 barrels of cement returned to surface. Company Man Doug Bennett and Cement supervisor Steven Long onsite.
0172985037 2018-04-24 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Precision Rathole augering a 36 inch diameter hole at a depth of 74 feet. According to the driller the hole is completely dry and consists of hard rock 6 feet below surface and below.
0521040768 2018-09-26 Plug / Plug Back No Violations On the day of my inspection I witnessed CJ/Otex fill the well bore with class A neat cement weighing 15.6 ppg and a yield of 1.18. Identified myself to company man Doug Bennett. Per my notes from the job, the 18.625 inch diameter mine string (87.5#/ft) was set at 272 feet KB. This should be 260 feet ground level. The shoe track was 23 feet long and float collar should be around 237 feet ground level. 2 inch diameter PVC was ran into wellbore and tagged at 222 feet ground level. Cementers pumped all 350 sacks of cement with good cement to surface. They also filled the mousehole with 3.5 yards of 9-sack grout. Casing was cut 30 inches below grade and a cap was placed on top with the permit number.
