| The purpose of my inspection was a status check. During my inspection the well was equipped with 8.625-inch diameter surface casing with a casing head, 4.5-inch diameter production casing with a tubing head, 2.375-inch diameter tubing, rods, and a pumping unit. The production valves appeared to be in the open position. The production equipment consisted of two 100-barrel tanks, a 30-barrel poly tank, one vertical separator, and a sales meter. The sales meter had a 2-inch diameter sales line, .1825-inch orifice plate, and a chart recorder. The sales line was plumbed into a pipeline labeled Dominion Gas and the valve was in the open position. The identification was located on the 100 barrel tank and the production records were current.

| On the day of my inspection I witnessed the production equipment included 2-100bbl tanks, a 50bbl polymer brine tank, a vertical separator, a drip, a 2 inch sales meter with a .1825 inch orifice and a chart recorder all within an earthen dike. The well was equipped with 8.625 inch casing, 4.5 inch casing, 2.375 inch tubing, rods and an Oilwell pumping unit powered by electricity with valves in the open position on the tubing and closed on the casing. Identification was present and production records were current.

During my inspection the well was equipped with 8.625-inch diameter surface casing with a casing head, 4.5-inch diameter production casing with a tubing head, 2.375-inch diameter tubing, rods, and a pumping unit. The production valves appeared to be in the open position. The production equipment consisted of two 100-barrel tanks, a 30-barrel poly tank, one vertical separator, and a sales meter. The sales meter had a 2-inch diameter sales line, .1825-inch orifice plate, and a chart recorder. The sales line was plumbed into a pipeline labeled Dominion Gas and the valve was in the open position. The identification was located on the 100 barrel tank and the production records were current.
