I was on location to conduct a preliminary restoration inspection.  The #8H well is in production and the well consists of a plunger lift assembly.  Jersey barriers were installed around the wellhead.  The well is identified with a legibly-marked well identification sign.  Identification was posted at the tank battery which consisted of eight 400 bbl production tanks, four labeled as containing produced water, and four labelled as containing condensate. The tank battery is surrounded by a steel dike.  There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters.  There is a truck grounding area for trucks collecting condensate and/or produced water.  Lightning protection and a windsock are installed on top of the production tanks.  Production equipment on the well pad consisted of:

My inspection revealed that the contractor is working on the lease road entrance along State Route 164 and rock trucks are hauling / spreading stone on the lease road.  I didn't inspect the well site at this time because of all the construction activity at the lease road entrance and there was no where to park the vehicle, so I wasn't parked on or along State Route 164. I was advised earlier in the week that a conductor rig should be on site sometime toward the end of this week.

| I was on site to witness the cementing of the 13 3/8" surface casing. My inspection revealed that Patterson Rig # 328 has reached a total depth of 490' and ran 484' of 13 3/8" surface casing. Five centralizers were installed on the casing string (on joints 1,2,4,7,10). O-TEX cemented the casing with 450 sacks of Class "H" cement ( 2% CaCl, .27% Cello-flake, and .4% C-41P). The cement density was 15.6 pounds per gallon with a yield of 1.2 cubic feet per sack. There was good circulation throughout the job and 18 barrels of cement was circulated to surface. Total displacement to land the plug was 68 barrels of freshwater.

I was on location to conduct a preliminary restoration inspection. The #8H well is in production and the well consists of a plunger lift assembly. Jersey barriers were installed around the wellhead. The well is identified with a legibly-marked well identification sign. Identification was posted at the tank battery which consisted of eight 400 bbl production tanks, four labeled as containing produced water, and four labelled as containing condensate. The tank battery is surrounded by a steel dike. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of: 6 producing wells 8- 400 bbl production tanks, four labelled as containing fresh water, and four labelled as containing condensate 6 sand separators 6 gas processing units 1 low pressure vertical separator 1 vapor recovery unit 1 horizontal separator

Jersey barriers are installed around the production equipment. The access road and man doors were locked. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road was stable in in good repair.

1342785218 2012-07-18 Drill / Deepen / Reopen No Violations I was on site to check the status of the construction of the well site / lease road. My inspection revealed that the contractor is working on the lease road entrance along State Route 164 and rock trucks are hauling / spreading stone on the lease road. I didnt inspect the well site at this time because of all the construction activity at the lease road entrance and there was no where to park the vehicle, so I wasnt parked on or along State Route 164. I was advised earlier in the week that a conductor rig should be on site sometime toward the end of this week.
1344834120 2012-08-13 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 surface casing. My inspection revealed that Patterson Rig # 328 has reached a total depth of 490 and ran 484 of 13 3/8 surface casing. Five centralizers were installed on the casing string (on joints 1,2,4,7,10). O-TEX cemented the casing with 450 sacks of Class H cement ( 2% CaCl, .27% Cello-flake, and .4% C-41P). The cement density was 15.6 pounds per gallon with a yield of 1.2 cubic feet per sack. There was good circulation throughout the job and 18 barrels of cement was circulated to surface. Total displacement to land the plug was 68 barrels of freshwater.
1345116253 2012-08-16 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 9 5/8 intermediate casing string. My inspection revealed that Patterson Rig # 328 has drilled a 12.25 hole to a total depth of 1754 and ran 1750 of 9 5/8 casing with 14 centralizers installed on the casing string (on joints 1,2,5,8,11,14,17,20,23,26,29,32,35,38). O-TEX cemented the casing with 630 sacks of ClassH 10% NaCl , .27% Cello-Flake , 4/10% C-41P, 1/10% C-35.There was good circulation throughout the job and approximately 24 barrels of cement circulated to surface. No violations noted during this inspection.
1345753830 2012-08-23 Drill / Deepen / Reopen No Violations I was on site to check the status of Patterson Drilling Rig # 328. My inspection revealed that Patterson Rig# 328 is drilling the lateral section of the well at 10,862 feet. They should reach total depth on this well sometime this weekend. I walked the location and no violations were noted at this time.
