+ | | I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. They used 5.2 gallons of freshwater per sack mix water. O-TEX pumped 39.93 barrels of cement and displaced the cement with 38 barrels of freshwater. There was approximately 6 barrels of cement returns to surface. My inspection of the well site revealed no violations at this time. |

The surface/intermediate annular gauge displayed a reading of zero psi, and the intermediate/production annular gauge displayed 243 psi. I performed a walk-around inspection of the production pad and erosional controls appeared functioning. |
0370082699 | 2015-11-16 | Preliminary Restoration | No Violations | I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:

They were currently slide drilling at 11700 feet in the lateral of their current well. They were having issues making good progress in the lateral and keeping the well bore straight. 1357583880 | 2013-01-07 | Drill / Deepen / Reopen | No Violations | I was on site to witness the cementing of the 20 inch conductor casing. My inspection revealed that Precision Rathole spudded the well on January 4th and ran the casing on January 5th. Precision drilled a 30 inch to a depth of 20 and then drilled a 24 inch hole to a depth of 112 feet. Precision ran 112 feet of 20 inch casing into the wellbore. O-TEX cemented the casing with 190 sacks of ClassA cement with 2% CaCl, .27% Cello-Flake, and 4/10% C-41P. The cement had a density of 15.6 pounds per gallon and a yield of 1.18 cubic feet per sack. They used 5.2 gallons of freshwater per sack mix water. O-TEX pumped 39.93 barrels of cement and displaced the cement with 38 barrels of freshwater. There was approximately 6 barrels of cement returns to surface. My inspection of the well site revealed no violations at this time. |
