| I was on location with Inspector Kevin Perry to inspect Sidewinder Rig #64.  Sidewinder was tripping in the hole with the bottom hole assembly to spud the surface hole.  We met on the rig floor with Herb (Drilling Supervisor) and discussed the new rig.  We inspected the backyard, mud pits, and shale shakers.  All equipment was on contaiment and the site was clean.  Sidewinder continued to trip into the hole.  No violations.

| I was on location to check the status of Sidewinder Rig #64.  Assembly of the drilling rig was nearing completion.  I met with Bill Ingalls (Mountaineer) and discussed a time frame for spudding the well.  Bill thought it would be this Sunday or Monday.  I inspected the location and drilling equipment.  The drilling equipment was fitted with mufflers to mitigate noise.  The rig is electric and generates power with housed diesel generators.  All equipment is spotted on containmet.  No violations.

| I was on location to check the status of this horizontal well.  I met with Bill Ingalls (Mountaineer) in the doghouse.  Sidewinder Drilling had drilled the 7.875 hole to a total measured depth (TMD) of 14,509' measured from the kelly bushing.  They were tripping the bottom hole assembly (BHA) out of the hole and were just laying down the PDC bit on the derrick floor.  The teeth on the upper sides of the bit were well worn.  I inspected the 5.5 casing and determined that it was new, P-110, and 20 lbs/ft.  I reviewed the cementing plans and their goal was to cement 1300' back  into the intermediate casing.  Bill thought it would be at least 24 hours to get the casing run into the hole.  Frank's Casing crew was on location to run the long string.  All drilling equipment is on containment.  No violations.

| I arrived on location at 3:56am and met with Inspector Kevin Perry to witness the cementing of the 9.625 intermediate casing.  Sidewinder Drilling was in the process of running casing and Universal Well Services was on location waiting to get rigged up.  Sidewinder ran 94 joints of casing with 31 centralizers, a float shoe on the bottom, and a float collar at 4186'.  Universal rigged up, established circulation, mixed and pumped 966 sacks of cement, and displaced the cement with 317 barrels of freshwater.  30-40 barrels of cement returned to the surface.  See cement/casing ticket for more details on this job.  No violations.

| I was on location at 9:40pm to witness the plugging back of the pilot hole and setting of the kick-off plug for this horizontal well.  The 7.875 pilot hole was drilled to a depth of 6300' (KB) into the Trenton/Black River Formations to allow for logging of the open hole.  Universal Well Services was on location to mix and pump the cement for this 2-stage plug back.  Plug #1 was set from 6225'-5100' to cover the Trenton/Utica section.  Plug # 2 was set from 5100'-4400' to serve as the kick-off plug for the horizontal leg of the well.  See plugging report for more details on this job.  No violations.

| I was on location with Tom Hill and intern Matt Waugh to check the status of this horizontal well pad.  All drilling equipment has been removed from location.  The wellhead is fenced, chained, and locked.  All annuli have pressure gauges and the annulus between the 9.625 intermediate casing and the 5.5 production casing had about 50psi on the gauge and is within acceptable limits.  The other gauges indicated no pressure.  This well has not been stimulated as of this date.  The pad is clean and drainage from the pad outside of the northeast and southeast corners is adequate.  Vegitation is well established at the drainage areas and around the location.  There is no off-site sedimentation or erosion occurring from the pad.  Preliminary restoration is approved.  No violations.

| I conducted a status check inspection of this Utica Shale well.  This hole was cored, plugged back, and the horizontal leg of the well was completed and 5.5 production casing was cemented on 3/15/2013.  The cased hole was never perforated or stimulated for production purposes.  The lease road access gate is locked and the well and company identification are posted at the entrance.  The wellhead is contained in a liquid tight cellar that is full of water.  The cellar is surrounded by multiple layers of orange safety fencing and concrete barriers are protecting the cellar and wellhead.  The rat hole inside the cellar needs to be plugged.  There is no surface production equipment on the well pad.  I will speak with Mountaineer Keystone about their plans for the well and ask that they apply for temporary inactive status (1st year) to comply with Chapter 1509.062 of the Ohio Revised Code.  No violations.

| I inspected this location to check the status of site construction and to collect new GPS readings of the well location.  Mountaineer Keystone is in the process of re-permitting the wells on this pad and have re-staked the well locations 30 to the south of the previous locations and have reversed the numbering of the wells.  The entrance and location have been stoned and the topsoil has been placed in a berm around the location and seeded.  Compost filter socks have been staked outside the berms to prevent erosion around drainage areas.  Site construction appears to be complete.  No violations.

| I was on location to check the status of Sidewinder Rig #64.  Assembly of the drilling rig was nearing completion.  I met with Bill Ingalls (Mountaineer) and discussed a time frame for spudding the well.  Bill thought it would be this Sunday or Monday.  I inspected the location and drilling equipment.  The drilling equipment was fitted with mufflers to mitigate noise.  The rig is electric and generates power with housed diesel generators.  All equipment is spotted on containmet.  No violations.

| I was on location to check the drilling status of this horizontal well.  I met with Seth Snider (Mountaineer Keystone) to discuss the drilling progress.  Sidewinder Drilling was currently drilling the horizontal leg at a measured depth of 9039' at roughly 40'/hr.  No violations.

| I was on location to check the status of this horizontal well.  I met with Bill Ingalls (Mountaineer) in the doghouse.  Sidewinder Drilling had drilled the 7.875 hole to a total measured depth (TMD) of 14,509 measured from the kelly bushing.  They were tripping the bottom hole assembly (BHA) out of the hole and were just laying down the PDC bit on the derrick floor.  The teeth on the upper sides of the bit were well worn.  I inspected the 5.5 casing and determined that it was new, P-110, and 20 lbs/ft.  I reviewed the cementing plans and their goal was to cement 1300 back  into the intermediate casing.  Bill thought it would be at least 24 hours to get the casing run into the hole.  Franks Casing crew was on location to run the long string.  All drilling equipment is on containment.  No violations.

| I inspected this location to check the status of site construction and to collect new GPS readings of the well locationMountaineer Keystone is in the process of re-permitting the wells on this pad and have re-staked the well locations 30' to the south of the previous locations and have reversed the numbering of the wells.  The entrance and location have been stoned and the topsoil has been placed in a berm around the location and seededCompost filter socks have been staked outside the berms to prevent erosion around drainage areasSite construction appears to be complete.  No violations.

| I arrived on location at 3:56am and met with Inspector Kevin Perry to witness the cementing of the 9.625 intermediate casing.  Sidewinder Drilling was in the process of running casing and Universal Well Services was on location waiting to get rigged up.  Sidewinder ran 94 joints of casing with 31 centralizers, a float shoe on the bottom, and a float collar at 4186.  Universal rigged up, established circulation, mixed and pumped 966 sacks of cement, and displaced the cement with 317 barrels of freshwater.  30-40 barrels of cement returned to the surface.  See cement/casing ticket for more details on this job.  No violations.

| I was on this well pad to check the status of the location since moving the drilling rig from location.  All drilling equipment has been removed from the location as well as all housing trailers.  Containment is still down on the drilling pad and around the wellhead.  The wellhead and cellar are surrounded by orange safety fencing.  All annuli on the wellhead are open and equipped with gauges which indicate no annular pressure.  No violations.

0080011625 2013-08-15 Preliminary Restoration No Violations I was on location with Tom Hill and intern Matt Waugh to check the status of this horizontal well pad. All drilling equipment has been removed from location. The wellhead is fenced, chained, and locked. All annuli have pressure gauges and the annulus between the 9.625 intermediate casing and the 5.5 production casing had about 50psi on the gauge and is within acceptable limits. The other gauges indicated no pressure. This well has not been stimulated as of this date. The pad is clean and drainage from the pad outside of the northeast and southeast corners is adequate. Vegitation is well established at the drainage areas and around the location. There is no off-site sedimentation or erosion occurring from the pad. Preliminary restoration is approved. No violations.
0200673967 2015-04-29 Production Wells No Violations I conducted a status check inspection of this Utica Shale well. This hole was cored, plugged back, and the horizontal leg of the well was completed and 5.5 production casing was cemented on 3/15/2013. The cased hole was never perforated or stimulated for production purposes. The lease road access gate is locked and the well and company identification are posted at the entrance. The wellhead is contained in a liquid tight cellar that is full of water. The cellar is surrounded by multiple layers of orange safety fencing and concrete barriers are protecting the cellar and wellhead. The rat hole inside the cellar needs to be plugged. There is no surface production equipment on the well pad. I will speak with Mountaineer Keystone about their plans for the well and ask that they apply for temporary inactive status (1st year) to comply with Chapter 1509.062 of the Ohio Revised Code. No violations.
0266577973 2013-02-25 Plug / Plug Back No Violations I was on location at 9:40pm to witness the plugging back of the pilot hole and setting of the kick-off plug for this horizontal well. The 7.875 pilot hole was drilled to a depth of 6300 (KB) into the Trenton/Black River Formations to allow for logging of the open hole. Universal Well Services was on location to mix and pump the cement for this 2-stage plug back. Plug #1 was set from 6225-5100 to cover the Trenton/Utica section. Plug # 2 was set from 5100-4400 to serve as the kick-off plug for the horizontal leg of the well. See plugging report for more details on this job. No violations.
0445346337 2013-02-12 Drill / Deepen / Reopen No Violations I was on location at 6:00pm to witness the cementing of the surface casing. Sidewinder rig #64 drilled a 17.5 hole to a depth of 510 and ran 484 of 13.375 surface casing into the hole. They were having trouble hooking up the rotating cement head and had to lay the tool down and replace the top joint of casing due to damaged threads. They picked the tool back up and threaded it into the casing. Universal rigged up to the rotating head and established circulation. They mixed and pumped 475 sacks of Type 1 cement and displaced the cement and plug to 438 with 68 barrels of freshwater. The plug landed at 231psi and the pressure was increased to 700psi and held for 5 minutes. They will wait 10 hours before breaking off and run a bond log. No violations. Sclumberger was rigging down the logging truck when I arrived on location. I met with Bill Ingalls (Mountaineer) and reviewed the bond log. The bond was good for the entire length of pipe. Black Bear Energy Services was preparing to cut off the casing and install and test the 13.625 stack and blow out preventor. No violations.
0754019759 2013-03-15 Drill / Deepen / Reopen No Violations I was on location at 1:30 A.M. to witness the cementing of the 5.5 production casing. I met with Bill Ingalls, Ross Schweitzer, and Jim Watson (Mountaineer) prior to the job. The goal was to get the top of the cement 1300 up into the 9.625 intermediate casing. Universal rigged up, established circulation, then mixed and pumped 1691 sacks of Type I cement. The plug was displaced and landed at 4:03 A.M. The pressure was increased to 2586 psi and held for four minutes before being released. All drilling equipment was on containment. No violations. See casing ticket for more details on this job.

We inspected the backyard, mud pits, and shale shakers. All equipment was on contaiment and the site was clean. Sidewinder continued to trip into the hole. No violations.
