| I was onsite to witness the cementing of the 13 3/8 casing set to 883 feet. The well was taken on large amounts of water during drilling but the hole was well conditioned before the cement was pumped by increasing circulation time and adding floseal. The job was pumped as planned with the cement weighed at 15.6ppg. There was a total of 45 barrels of cement returned to surface and the floats held at 875psi for 5 minutes with little fluid coming back. They are going to set their 9 5/8 casing around 1500'.

| I was at the site checking the drilling progress and obtaining cementing paperwork from the surface and intermediate cementing operations.  I met with Larry Williamson, drilling consultant.  The rig was in the process of drilling the curve at a total measured depth of 7,555' KB at 12:15 pm.

| I was onsite to check the status of the drilling of the 17.5 surface casing hole. They are currently at 669' and are taking on 150 barrels of water with a chlorides level of 2800. They are concerned whether or not to set casing at their current location in the Big Injun due to the on take of water. Brett Farnsworth and Supervisor Tom Hill were contacted and decided to set casing at 885 feet, 15 feet short of the 900' permitted depth. The cement job should occur later tonight/mourning.

They are concerned whether or not to set casing at their current location in the Big Injun due to the on take of water. Brett Farnsworth and Supervisor Tom Hill were contacted and decided to set casing at 885 feet, 15 feet short of the 900 permitted depth. The cement job should occur later tonight/mourning.

| I was onsite to witness the cementing of the 13 3/8 casing set to 883 feet. The well was taken on large amounts of water during drilling but the hole was well conditioned before the cement was pumped by increasing circulation time and adding floseal. The job was pumped as planned with the cement weighed at 15.6ppg. There was a total of 45 barrels of cement returned to surface and the floats held at 875psi for 5 minutes with little fluid coming back. They are going to set their 9 5/8 casing around 1500.

-based mud used to drill and condition the hole, a lead cement followed by a tail cement were pumped.  The lead cement was mudscaled at 13.5 pounds per gallon (ppg).  Schlumberger pumped 302 bbls of lead cement at 13.5 ppg.  The tail cement was mudscaled at 15.0 ppg.  Schlumberger pumped 261 bbls of tail cement at 15.0 ppg.  The plug was dropped and displaced with 284 bbls of water as measured by pump truck tanks.  The plug bumped the float collar at 4:48am on 05/29/2013.  The plug landing pressure was 2700 psi, and it was raised to 3220 psi, and held for 5 minutes.  Pressure held.  The float held when the pressure was released.  3.0 bbls of water bled back.  There was good circulation during the Mudpush spacer, the lead and tail slurries, and the first 110 bbls or so of the displacement.  After about 110 bbls into the displacement, the pumping pressure began to climb rapidly, and the flow appeared like it was starting to bridge off down hole.  Schlumberger worked the pump rate up and down, and was able to continue pumping. Several times during the pumping, the pressure increased significantly, and the pump rate was slowed or temporarily stopped. | I was on location to conduct a preliminary restoration inspection.  The well is a plunger lift unit.  Jersey barriers were installed around the wellhead.  The well is properly identified with a clear, legibly-marked well identification sign.  The tank battery consisted of six 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment.  There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators.  There is a truck grounding area for trucks collecting condensate and/or produced water.  Lightning protection and a windsock are installed on top of the production tanks.  Other production equipment on the well pad consisted of:

Jersey barriers are installed around the production equipment.  The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked.  Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation.  Drainage and erosion controls at the pad are in place and functioning.  A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure.  The pad has a crushed limestone surface, and is in good repair.  The lease road is stable and drivable.  Preliminary restoration passed.

They are currently at 669 and are taking on 150 barrels of water with a chlorides level of 2800. They are concerned whether or not to set casing at their current location in the Big Injun due to the on take of water. Brett Farnsworth and Supervisor Tom Hill were contacted and decided to set casing at 885 feet, 15 feet short of the 900 permitted depth. The cement job should occur later tonight/mourning.
0998554329 2017-07-12 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The #8H well is in production and is equipped with a plunger-lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and all man doors were locked.

The equipment on the pad consists of the following: 2 producing wells

2 Low pressure vertical separators 2 sand separators 1 gas processing unit 1 vapor recovery unit 1 heat treater

The access road was stable. No violations were noted at the time of inspection.

1316841479 2013-05-17 Casing String Witnessed No Violations I was onsite to witness the cementing of the 13 3/8 casing set to 883 feet. The well was taken on large amounts of water during drilling but the hole was well conditioned before the cement was pumped by increasing circulation time and adding floseal. The job was pumped as planned with the cement weighed at 15.6ppg. There was a total of 45 barrels of cement returned to surface and the floats held at 875psi for 5 minutes with little fluid coming back. They are going to set their 9 5/8 casing around 1500.
1369760847 2013-05-24 Drill / Deepen / Reopen No Violations I was at the site checking the drilling progress and obtaining cementing paperwork from the surface and intermediate cementing operations. I met with Larry Williamson, drilling consultant. The rig was in the process of drilling the curve at a total measured depth of 7,555 KB at 12:15 pm.
1370538125 2013-05-29 Casing String Witnessed No Violations

-based mud used to drill and condition the hole, a lead cement followed by a tail cement were pumped. The lead cement was mudscaled at 13.5 pounds per gallon (ppg). Schlumberger pumped 302 bbls of lead cement at 13.5 ppg. The tail cement was mudscaled at 15.0 ppg. Schlumberger pumped 261 bbls of tail cement at 15.0 ppg. The plug was dropped and displaced with 284 bbls of water as measured by pump truck tanks. The plug bumped the float collar at 4:48am on 05/29/2013. The plug landing pressure was 2700 psi, and it was raised to 3220 psi, and held for 5 minutes. Pressure held. The float held when the pressure was released. 3.0 bbls of water bled back. There was good circulation during the Mudpush spacer, the lead and tail slurries, and the first 110 bbls or so of the displacement. After about 110 bbls into the displacement, the pumping pressure began to climb rapidly, and the flow appeared like it was starting to bridge off down hole. Schlumberger worked the pump rate up and down, and was able to continue pumping. Several times during the pumping, the pressure increased significantly, and the pump rate was slowed or temporarily stopped. Circulation was intermittently lost during peak moments of pressure, and then regained when the pressure dropped. Circul

1739914096 2015-10-20 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.
