1983895 2011-06-15 No Violations Noted Site visit was in response to ARM Inspector Dennis Graham phone call.

A request was made to move a culvert and deviate from the prepared plan. The movement of the culvert was to allow access for the landowner access to a hay storage area. The replacement of the culvert put the structure in what appeared to be a better position to naturally channel water from the site. On-site plans were marked, ok﾿ed and initialed. No violations noted at this time.

1985331 2011-07-01 No Violations Noted Site visit was in response to OG-57 waiver for construction within 100 feet of a wetland.

The pad and access road will encroach on this restriction. Dennis Graham (ARM) and Steve Greer (ARM) were on site during this inspection. It was stressed the BMPs be strengthened on areas that border the wetlands. Waiver is approved. No violations noted

2069256 2012-05-17 No Violations Noted At the time of inspection the well had not been drilled. No observation of accelerated erosion in relation to the well pad or access road. The E&S plan was onsite and properly installed and maintained.
