| An inspection of the Matz 824 was conducted on October 11, 2012 in response to a Notice of Termination and six Well Site Restoration Reports received by the Department. 
 
Several erosion control issues were noted during this inspection.  Sediment is accumulation in the rock-lined ditch at the left side of the entrance to the pad.  The north side of the cut slope to the east of the pad does not meet the requirements of 25 Pa Code §102.22(a)(2) (i) Permanent Stabilization.  A minimum uniform 70% perennial vegetative cover, with a density capable of resisting accelerated erosion and sedimentation was not observed.  These areas must be stabilized. The rock-lined ditches and two sediment traps (ST2 and ST-3) remain on site.  These structures must be identified in the Post Construction Stormwater Management Plan along with any maintenance requirements.  The above items must be addressed prior to the approval of the Notice of Termination or Well Restoration Reports. - MRN

| An inspection of the Matz 824 site in Chatham Township, Tioga County was conducted on September 5, 2013.  The last inspection conducted on behalf of the operator was on August 19, 2013.  1H, 2H, 3H, 4H, 5H and 6H are drilled and in production.  The site is well vegetated and stabilized.  No puddles exhibiting elevated conductivity were observed at this time.  The Post Construction BMPs listed in the E&S plan have been installed.  The Restoration reports for the 1H through 6H well are being approved.    No violations were cited during this inspection.

2106986 2012-10-11 No Violations Noted An inspection of the Matz 824 was conducted on October 11, 2012 in response to a Notice of Termination and six Well Site Restoration Reports received by the Department.

Several erosion control issues were noted during this inspection. Sediment is accumulation in the rock-lined ditch at the left side of the entrance to the pad. The north side of the cut slope to the east of the pad does not meet the requirements of 25 Pa Code ᄃ102.22(a)(2) (i) Permanent Stabilization. A minimum uniform 70% perennial vegetative cover, with a density capable of resisting accelerated erosion and sedimentation was not observed. These areas must be stabilized. The rock-lined ditches and two sediment traps (ST2 and ST-3) remain on site. These structures must be identified in the Post Construction Stormwater Management Plan along with any maintenance requirements. The above items must be addressed prior to the approval of the Notice of Termination or Well Restoration Reports. - MRN

2202853 2013-09-05 No Violations Noted An inspection of the Matz 824 site in Chatham Township, Tioga County was conducted on September 5, 2013. The last inspection conducted on behalf of the operator was on August 19, 2013. 1H, 2H, 3H, 4H, 5H and 6H are drilled and in production. The site is well vegetated and stabilized. No puddles exhibiting elevated conductivity were observed at this time. The Post Construction BMPs listed in the E&S plan have been installed. The Restoration reports for the 1H through 6H well are being approved. No violations were cited during this inspection.
