| I&O referral inspection. 11" steel casing. Fluid measured with tape measure at 24'11". No oil was observed on the end of the tape or weight. Fluid was observed before the measurement and no bubbles were seen. No gas detected at well site. Well is 43' from the edge of CR 29 (Paddle Creek Rd). Photos and measurements were taken of site and obstructions. There were no overhead power lines over either the well or the road.

| Orphan Well Plug job. Contractor Barry Bowers grouted well to surface with 4 yards 9 sack grout. Grout was brought to location in a ready mix truck. The grout was then dumped into a hopper on a cement pump truck. The cement pump truck had extended a hydraulic arm to reach the well casing from the road. A 1 inch plastic siphon string was ran into the casing to bottom. As grout was dropped into well casing, the water was displaced through the siphon string and into a steel container at the road. Bower's water truck vacuumed the water out of the steel container. All fluids were contained during the plug job. Casing was cut off  30 inches below grade and monument installed on casing. Backfilled the hole and graded location. All excavation was done by hand shovels.

| Traveled with Jon Scott to the Knox Energy's S & D Enterprises #1D3-30-3540 (API 34079202620000). Stopped at this orphan well to check out the final restoration of this well located in Lake Vesuvius Park, Wayne National Forest.  Looks great, very little disturbance noticed.
