I spoke with Kyle Ahbel from Enervest about what needs to be done before final restoration can be passed. The flow line will be removed and all affected areas will be planted with clover or alfalfa mix seed. The lease road was being used for access to the farmers fields before the well was installed. No violations found.

He advised me he would be done removing the flow line today and would have all of the excavation work finished. The seed and straw would be done by the afternoon on 10/1/2014. After vegetation is established I will pass final restoration.

| I was on location to witness the remainder of this plug job. Nick's Well Plugging set the Lime plug and the Surface plug with my permission without being present. I was on location to witness the top plug. Nicks rigged up and broke circulation and set 62 sacks of Class A cement to the top of the hole. Cement circulated to surface. The remaining 5 joints of 4.5 casing was removed from the well and the hole was topped off with cement. The GPS Coordinates are +/-7'.

| I responded to a call from the landowner concerning a possible oil and gas leak on a transmission line.  While in route, I called and instructed Dan Reagan, well tender for Enervest, to meet me on location to assess the situation.  Once I arrived on location, I walked the entire location with Mr. Reagan and the landowner.  During my investigation, I found the ground saturated with water.  I observed several, small pools of stagnant water with a biological film and decaying leaves and grass.  The topography in the saturated location is highly sloped in a west to east orientation.  The landowner stated the area has a natural spring.  Upon further inspection, I found no oil or gas spill or leak.

The tank battery consists of a 100 BBL production tank, a 100 BBL brine tank, and a mechanical separator. The tank battery is over grown with weeds. The proper identification is on the tank battery. The lease road is in stable condition. The last reported year of production was 1999.  I will contact Mr. McDaniel, a supervisor for Enervest about the well. The well shall be placed back into production in 90 days.  If the well is not producible a plugging permit shall be obtained within 90 days and the well shall be plugged within 180 days of this compliance action.

The plug was displaced and the well was shut-in until the following morning. No violations.
