| My inspection found the well equipped with a plunger lift unit surrounded by overgrown weeds.  It had no identification sign.    The tank battery contained one, 100 barrel production tank.  The identification sign stenciled onto the tank with half of the proper information.  There was no plugs on the load lines.  The dike was weathered; however, it meets containment requirements.  The separator was in good working condition.  There was one, 50 barrel brine tank next to a vertical drip tank at the sales flow line.  The 7 day production chart was located next to the sales flow line.  The well shares the production tank and separator with permit number 2-1668.  The lease road was in stable condition with a locked wire steel line at the entrance.  This well was placed in production prior to House Bill 278.  I called Jamie Campbell at 440-858-7252.  I instructed him to install the plugs on the load lines, remove enough weeds to expose the wellhead,  and post the identification sign.  I will re-inspect by 2/8/2013.

| I received a letter in reference to complaint ID #2500 from Attorney Thomas Carey, Jr. representing the landowner, who had concerns about the condition of a well site on his property.  My inspection found the well was in production.  The tank battery, separator, vertical separator, and both wells were engulfed with overgrown weeds.  There were small trees growing next to the flow line and stairs at the production tank and around the well.  I had to tie orange tape to mark the location of the well.  I researched our database and found the annual production reports were up to date.  I called Valerie Dunn of Well Completion Services at 440-858-2343.  I informed her a complaint was filed concerning the condition of the well site.  I told her to have the weeds and small trees cut down at the tank battery, separators, and both wells by 10/23/2013.  I noticed a plug was placed on the load line and the stenciled identification sign had all of the correct information per my request on 1/28/2013.  I took photos of the entire well site.
