| I inspected this unit pursuant to an earlier complaint regarding the preliminary restoration. The unit consists of a mechanical plunger lift at the wellhead, enclosed in a 6-foot, board-on-board enclosure. The tank battery (TB) is located 50 feet to the east and is within an 8-foot board-on-board enclosure. There are three 8-foot pine trees on the west side of the TB enclosure. There are several pine trees south of the wellhead, but nothing surrounding the unit. I will contact Seagull Development and inform them that screening will be required around the wellhead as stipulated by the permit conditions.

| I observed the wellhead, which consists of a mechanical plunger-lift unit enclosed within a slat-board fence.  The fence is locked. The tank battery (TB) is located 75 feet to the east. The TB is also enclosed within a wood-slat fence, which is approximately 8 feet high. The TB is comprised of one, 100-barrel (BBL) steel production tank and one, 10-barrel poly brine tank. The domed top portion of the poly tank is somewhat depressed downward, but is otherwise in good condition. The remaining mechanical appurtenances include a mechanical separator, and standard valves. The production tank is fitted with a charcoal canister filter, a spark arrestor, and a low-pressure relief valve as required. The interior of the TB is enclosed within an earthen dike. The TB interior area is gravel lined and has been recently weeded. Unit appears to be in good working order and producing. There is an ID sign on the TB, but not at the wellhead unit. A prior inspection had noted the presence of production tubing and casing along the lease road. These materials have been removed and the lease road appears to be well maintained. While I was onsite, contractors for the owner arrived to repair the brine tank. The contractors were successful at restoring the top of the poly tank to the rounded shape. I discussed with the contractors maybe putting the ID sign on the wellhead fence where it might be more visible. They indicated that they had been told previously to put the sign at the tank battery, possibly by the fire department. I observed the load-out couplings to be locked. I did not observe any evidence of active or historic leaks or releases of gas, oil, or brine in or around the unit. The contractors indicated that there is a current Spill Prevention Control and Countermeasures (SPCC) plan for the TB.

The owner has gone to the additional measures of applying 2" foam panels to abate the noise issues. No violations were found during the inspection and the well is in compliance with the laws of the State of Ohio.

Mr. Visnar stated there is an agreement between the company and the lease holders, along the north property line that several trees to be planted along the property line as a sound and visual barrier.

| I inspected this unit pursuant to an earlier complaint regarding the preliminary restoration. The unit consists of a mechanical plunger lift at the wellhead, enclosed in a 6-foot, board-on-board enclosure. The tank battery (TB) is located 50 feet to the east and is within an 8-foot board-on-board enclosure. There are three 8-foot pine trees on the west side of the TB enclosure. There are several pine trees south of the wellhead, but nothing surrounding the unit. I will contact Seagull Development and inform them that screening will be required around the wellhead as stipulated by the permit conditions.

| Due to a complaint concerning the brine tank being collapsed at this well , I enlisted the assistance of Steve Tompkins, ODNR inspector for Lake County to inspect this well for me. While in the process of doing so the maintenance crew for Seagull Development arrived on location and repaired the tank in question by raising the collapsed portion of the brine tank to original condition. The integrity of the tank was never compromised and no leaks or spills were discovered. The well is in compliance with 1501-9-9-05, urban well producing operations.

Asked Antoney company as to where the fluid came from he said from when company was swabbing the well this area was under where lines when to the frac tanks. Soil was then removed and area filled in with fresh fill.

1348700862 2012-09-26 I observed the wellhead, which consists of a mechanical plunger-lift unit enclosed within a slat-board fence. The fence is locked. The tank battery (TB) is located 75 feet to the east. The TB is also enclosed within a wood-slat fence, which is approximately 8 feet high. The TB is comprised of one, 100-barrel (BBL) steel production tank and one, 10-barrel poly brine tank. The domed top portion of the poly tank is somewhat depressed downward, but is otherwise in good condition. The remaining mechanical appurtenances include a mechanical separator, and standard valves. The production tank is fitted with a charcoal canister filter, a spark arrestor, and a low-pressure relief valve as required. The interior of the TB is enclosed within an earthen dike. The TB interior area is gravel lined and has been recently weeded. Unit appears to be in good working order and producing. There is an ID sign on the TB, but not at the wellhead unit. A prior inspection had noted the presence of production tubing and casing along the lease road. These materials have been removed and the lease road appears to be well maintained. While I was onsite, contractors for the owner arrived to repair the brine tank. The contractors were successful at restoring the top of the poly tank to the rounded shape. I discussed with the contractors maybe putting the ID sign on the wellhead fence where it might be more visible. They indicated that they had been told previously to put the sign at the tank battery, possibly by the fire department. I observed the load-out couplings to be locked. I did not observe any evidence of active or historic leaks or releases of gas, oil, or brine in or around the unit. The contractors indicated that there is a current Spill Prevention Control and Countermeasures (SPCC) plan for the TB.
1349279091 2011-05-13 Travelled to this well on May 13, 2011 with Inspector Norburt Lowder to investigate noise complaint #3123 issued 5/10/2011. Investigated well site to include well head, tank battery and sales lines. . Well is constructed under 1509: 9-9-05, producing operations, sub section Urban Producing Operations. The owner has gone to the additional measures of applying 2 foam panels to abate the noise issues. No violations were found during the inspection and the well is in compliance with the laws of the State of Ohio.
1350589669 2011-05-13 05-10-2011 Phone call to company and talked to John Visnar He said he will look into a solution to the noise from the tank battery (stated the problem is that when the well was turned on and flowing into the sales line it flowed 24 hour a day7days and it did not slow down!). Mr. Visnar stated there is an agreement between the company and the lease holders, along the north property line that several trees to be planted along the property line as a sound and visual barrier.
 Inspection on 05-13-2011 Ray Wallace(Inspector1)and Norburt Lowder (Inspector 3), Found landscape not done due to weather gave extenstion. A crew for the  company was working on sound proofing being placed on the inside of the perimeter fencing of the tank battery.
