| Inspected the Innis #1 (#104) as part of a standard productive well status check. The well is being pumped by an electric powered pump jack. Oil has leaked from the stuffing box and pooled around the well head and contaminated a patch approximately 5 foot wide by 17 foot long along the well pad. The storage consist of one silver 100 barrel tank #3277 and a 1000 gallon plastic brine tank. No identification is posted at the well or storage. No production has been reported to the State of Ohio, but two run tickets where left on location by Devco Oil. 1) 2/3/2012 #93464 and 2) 5/25/2011 #89497 and shows owner as William Hammett.
