Steel tank, as a vent line. Tank has apprx. 1 ft. of fluid in bottom. Fluid in  1 1/2inch line to the well head has unfrozen fluid in it.  7  digital photos taken.    Arrived at site 10:45am.

| Inspection revealed two (2) one (1) inch plastic lines. The first line is running from the 100 Bbl. Storage tank to the well head and the second line running from the well head to the 100 Bbl. Storage tank. I presume this is for the disposal of brine down the annular. I took  four (4) photos. I called and left a voice mail with Tom Domestic to see if this well is approved for Annular Disposal. My phone call was never returned. Supervisor Fry Contacted me and advised me that he had been on location and took moor photos and that this well was not approved for AD and he would be notifying the owner. 01/23/2002 At 1:48 P.M. Dave Hodges called me for the date I was on site. I advised 01/02/2002. At 2:22 P.M. Jeff Fry called and advised me that Mr. Murphy tolled him that me and him (Mr. Murphy ) had sat in the same truck and did a MIT on the well. I advised Jeff that my records showed this well was never permitted for AD and that we had tested the No.6 well that is across the road. As for my report I have been waiting for the information I requested. (Report filed 01/23/2002)
