| FOLLOW UP TO NOV #  1248i62  Free oil inside of two (2) tank dikes, and ID violation.
+
| Inspection revealed two (2) tank battery dikes with free oil inside them. Both had ran over and off location. The three (3) inch load line valve has a plug in it but is still leaking fluid. (Field test revealed the fluid is brine. Incomplete identification posted on the location. Took 12 photos and quantab fluid from three (3) inch valve on load line. (Field test showed chlorides was over 6266 ppm (mg/L).        OWNER NEEDS TO DO THE FOLLOWING BEFOR REMEDY DATE. 1) FILL OUT ID SIGN. 2) REPLACE THREE (3) INCH VALVE ON THE LOAD LINE. 3) REMOVE ALL FREE FLUIDS FROM DIKES.  4) REMOVE ALL CONTAMINATED SOIL AND DISPOSE OF IN A PROPPER MANNER. 5) REPLACE SOIL AND SEED, SOD, AND MULCH TO PREVENT EROSION AND SEDIMATION. SUPPLY THE DIVISION WITH THE NAME OF THE DISPOSAL SITE AND TICKETS FOR THE AMOUNT OF MATERIAL DISPOSED OF.    WEATHER CONDITIONS: Sunny and 18 degrees and ground is frozen at this time.

| Follow-up revealed the ID sign has been filled out and the three (3) inch valve on the load line has been replaced. All the free oil at both tank dikes have been removed bout fluids still remain. I field tested the fluids from the two (2) tank battery's and both are over the 250 Ppm standard. The owner had placed straw at the load line and the southern edge of the lower tank battery. I took four (4) photos showing the fluids and straw areas.  1- OWNER NEEDS TO REMOVE ALL FLUIDS FROM BOTH TANK BATTERY DIKES ASAP.  2- OWNER NEEDS TO KEEP DIKES FREE OF ALL FLUIDS.

1047387775 2003-03-11 Inspection revealed two (2) tank battery dikes with free oil inside them. 1050423921 2003-04-15 Follow-up revealed the ID sign has been filled out and the three (3) inch valve on the load line has been replaced.
1052316462 2003-05-07 Follow-up revealed the western part of the upper pit is full of fluid and is about to run over the edge. The lower dike has fluid in in and needs to be drained. OWNER NEEDS TO REMOVE ALL FLUIDS FROM THE UPPER AND LOWER DIKES AND BUILD UP THE SOUTHERN SIDE OF THE UPER PIT. WEATHER CONDITIONS: Cloudy with light rain, and 60 degrees.
1248i6251975 2001-05-08 Free oil inside of two (2) tank dikes, and ID violation.
13274106 2001-12-12 Follow-up revealed dike area has been cleaned up, and ID posted. Weather Cloudy and 45 Degrees.
1348498447 2012-08-29 I inspected this well today for John Lothes. I took a GPS reading at the well head to correct the well location.
