| Drainage controls have been installed.  Operator has installed silt fence on the north and south side of the lease road before the home.  Two sumps have been constructed, one at the south end of the drilling location, where the 36 inch culvert empties onto the property and a 2nd sump at the north end of the property.  The north sump is where the site discharges off of the property.  Both of the sumps had three 8 inch siphons installed in them.  Site had received heavy rain that night, the rain washed grease and oil from the air trailer.  Operator placed diapers and a boom around the trailer to collect the residual oil and grease coming from the trailer.  Pit; the north end of the pit the liner was not visible water level was up to the berm.  Physically confirmed that the liner was in place under the berm and water.  Driller had water trucks on location and ordered to remove water from the water tanks.  Pit water was pumped into water tanks to allow for additional free board.  Lease road was stable.  Drill depth was 2189 feet.  BOP was in place and chained down.

Fabish Fire Chief for North Royalton, Chief received a call from the Mayor's office to review site.  Reviewed drainage controls on site.  Operator had installed drainage controls in one location on site, drainage channel to the north of the drill location.  Silt fence and straw bales were not installed correctly.  Silt fence was not trenched in.  Straw bales were not staked in.  Drainage moved straw bails. Pond to the north of the drill location was brown in color.  The stream that receives drainage from the pond was cloudy.  Instructed operator to install  silt fence on the north and south side of the lease road, from State road to the home.  Install silt fence around topsoil stockpiles north of the home, down slope side.  Install a sump with a siphon dam at the north end of the location.  Repair silt fence in drainage channel and stake down straw bails or remove straw bales.  Reviewed with equipment operator the correct way to install silt fence and straw bales.  Mark Schmitzer City Engineer arrived on site, reviewed updated drainage plan with Mark.  Mark requested a 2nd sump be installed at the north end of the drill location.  A culvert discharges onto the property at that location.  Drainage flows south to north across the drill location.  Drainage flows from State Road east across the front of the location. Lease road was stable.  Safety fence was up around pits and site.

| My visit to the well site today was to inspect preparation for rig move on next week. Company is adding a third well to this location. I also inspected this well and found no violations. Well is in compliance. I was accompanied by Richard Bayless from the Columbus office.

The wellhead is equipped with a Linger Rod pump and is incased in a board on board fence and secured with locks. The pressure on the annulus is 200 PSI. The interior of the pump location is clean and free of debris. The tank battery consists of one 110 Barrel lowboy tank, one 50 barrel brine tank, a separator and a metering run. There are two load lines exiting the rear of the tank battery and are secured with bull plugs. Preliminary inspection passed.

The unit is producing. The loadouts are locked. There is secondary containment at the battery, ID placards at the wellhead and battery, and process safety equipment (low-pressure relief valve, spark arrestor, and carbon filter). Evergreen screening is present around the battery and screening has been placed at the top of the driveway/lease road. The internal spacing at the battery is in compliance with the rules. The 8.625-inch casing is currently buried. I contacted Ted with Ohio Valley Energy and advised him that the backside must be exposed and gauged or valved.

The tank battery is located near the road and is accessed from the neighboring drive on the north side. The wellhead and tank battery are located in separate board-on-board enclosures. The fittings and connections are secure and free from signs of leaks or problems. Secondary containment is clear and dry, Load-outs and gates are locked.
