| I inspected this well as part of routine inspection activity. The well is producing and production records are current. Entry gates are locked and secure, at both the wellhead and battery. Process safety equipment is present. Fittings and connections are secure and free from signs of leakage or problems. Tank hatches are closed and secure. The short access road is dry, level, and well drained with no signs of erosional deficit. At the time of this inspection, I observed the following deficiencies:  ID signage is lacking required information. The evergreen screening required as part of site restoration is incomplete (missing on the south side of the wellhead). Warning signage is incomplete.  I have notified Ohio Valley Energy that these deficiencies must be corrected as soon as practicable or by 06/29/2015.  Please contact Inspector Stephen Tompkins at 330-907-4647 for questions regarding this inspection.

| FOLLOW UP TO NOV #  1948330034 On 9/24/2011 Inspector Norburt Lowder received a call from Wade Balser of OEPA regarding a complaint he received from the Broadview Heights Fire Department of oil bubbling up in the back yard of 8490 Wyatt Road in Broadview Heights, Ohio. This property is owned by Irene Krotz and is located immediately south of the Kral 1D (#2187). Mr. Lowder then contacted Gene Chini and Ray Wallace of ODNR, Gonzoil and Mrs. Krotz about the problem. Mr. Wallace arrived at the site at  approximately 5:15 PM on 9/24/2011. He inspected the back yards southeast of the well site, (Krotz, Tremblay and Benes), and discovered evidence of petroleum on the grass in a small depression that ran through the Krotz and Tremblay back yards. He then investigated the Kral 1D drilling site and noticed the soil adjacent to the drilling rig air compressor and mud pumps was covered with "PeatZorb". There were small of motor oil on the ground surface which appeared to be flowing into a small pool of water that had collected on the east side of the location. Mr. Wallace also discovered some broken clay field tile adjacent to this pool of water and, upon further investigation, the opening to the solid clay feeder tile which appeared to run south along the east edge of the excavated portion of the drill site. It appears that the motor oil had made its way to the pool of water and into the drain tile. Two (2) holes were excavated by hand to locate the field tile outside of the drilling location. OEPA had straw bales placed around the second (farthest) excavated area and approved that work be terminated until Sunday, 9/25/11, due to darkness. On Sunday four (4) additional holes were excavated to locate the tile. Eventually the end of the field tile was found in what appeared to be a French drain located on the property line between Benes and Tremblay. There was a second drain tile that emptied into this French drain from the east. The first drain tile was flushed with fresh water and all water/oil collected was pumped back into the drilling pit. OEPA recommended that absorbent pads be placed in the tile and on top of the water in the French drain to absorb any residual oil flow. Excavations will be backfilled on Monday or Tuesday, weather permitting. Gonzoil will remediate and restore the affected area to original condition following well completion. Gonzoil will contact ODNR and OEPA once restoration is completed. No violation was issued to Gonzoil at this time. OEPA to determine if the water in the field tile is considered " Waters of the State". ODNR and OEPA also emphasized that the rig maintenance problem must be ratified.

| I inspected this producing unit as part of routine inspection activity. The wellhead pump jack enclosure is secure. Valves and connections are secure and free from signs of leakage or problems. At the time of my inspection, the area is covered with several inches of snow. Access to the well and battery  is from the church parking lot which is clear and snow has been plowed. Load-outs are bull-plugged and locked. I did not observe any problems at the time of this inspection.

Access to the well and battery is from the church parking lot which is clear and snow has been plowed. Load-outs are bull-plugged and locked. I did not observe any problems at the time of this inspection.
1948330034 2011-09-24 On 9/24/2011 Inspector Norburt Lowder received a call from Wade Balser of OEPA regarding a complaint he received from the Broadview Heights Fire Department of oil bubbling up in the back yard of 8490 Wyatt Road in Broadview Heights, Ohio. This property is owned by Irene Krotz and is located immediately south of the Kral 1D (#2187). Mr. Lowder then contacted Gene Chini and Ray Wallace of ODNR, Gonzoil and Mrs. Krotz about the problem. Mr. Wallace arrived at the site at approximately 5:15 PM on 9/24/2011. He inspected the back yards southeast of the well site, (Krotz, Tremblay and Benes), and discovered evidence of petroleum on the grass in a small depression that ran through the Krotz and Tremblay back yards. He then investigated the Kral 1D drilling site and noticed the soil adjacent to the drilling rig air compressor and mud pumps was covered with PeatZorb. There were small of motor oil on the ground surface which apppeared to be flowing into a small pool of water that had collected on the east side of the location. Mr. Wallace also discovered some broken clay field tile adjacent to this pool of water and, upon further investigation, the opening to the solid clay feeder tile which appeared to run south along the east edge of the excavated portion of the drill site. It appears that the motor oil had made its way to the pool of water and into the drain tile. Two (2) holes were excavated by hand to locate the field tile outside of the drilling location. OEPA had straw bales placed around the second (farthest) excavated area and approved that work be terminated until Sunday, 9/25/11, due to darkness. On Sunday four (4) additional holes were excavated to locate the tile. Eventually the end of the field tile was found in what apppeared to be a french drain located on the property line between Benes and Tremblay. There was a second drain tile that emptied into this french drain from the east. The first drain tile was flushed with fresh water and all water/oil collected was pumped back into the drilling pit. OEPA recommended that absorbent pads be placed in the tile and on top of the water in the french drain to absorb any residual oilflow. Excavations will be backfilled on Monday or Tuesday, weather permitting. Gonzoil will remediate and restore the affected area to original condition following well completion. Gonzoil will contact ODNR and OEPA once restoration is completed. No violation was issued to Gonzoil at this time. OEPA to determine if the water in the field tile is considered Waters of the State. ODNR and OEPA also emphasized that the rig maintenance problem must be ratified.
-2097895151 2013-10-10 The producing unit consists of a wellhead and aboveground storage tank (AST) battery in separate chain-link enclosures. The wellhead is a mechanical plunger lift unit and is producing. The battery is comprised of 1 140-barrel steel production tank. A 50-barrel poly brine tank, and separator. Secondary containment, spacing, and signage is good. Process safety equipment (spark arrestor, carbon filter, and low-pressure relief valve) are present. Gates and load-outs are locked. No problems noted.
