| Inspected production well on 05/19/2014. The tank battery consists of 2-100 bbl steel tanks and 2-vertical separators located within an earthen dike. The separator-end of the dike was full of water at the time inspection; a gas leak at the separator was noted. The well is a rabbit well. No identification is posted at either the tank battery or the wellhead.  Research into the production history of the well showed that no production has been filed since 1991, therefore the well must be plugged.

| Requested by Sherry Young to inspect well for status. Well is equipped with 4.5'' casing and a valve. 2-100 barrel tanks and separators in battery. No identification on tanks or at well head. Gas chart is old and faded. No commercial gas being sold, but landowner is using gas to home. Pressure gauge reads 280 psi at well head. Discussed well status with Jack Malusky. He advised it produces gas for home. He has had no contact with Bootstrap for years. I advised him Steve Harley is interested in evaluating well for production. Sun, windy, 30 degrees.
