| On site as the result of a phone conversation w/ Fred Jackson (614) 530-3459. 5-17-06 at approx 9:04 Am I received a call from Mr Jackson he wanted to know if flaring gas was pollution. I told him no and that burning a flair was legal in Ohio if there was no market for gas. He also wanted to know about fire hazards form burning the flair. Such as trees, I told him he would have to talk to the local fire department. He got mad and said that he appreciates how much his government is doing for him. Then he hung up on me.
 
The well is shut in at this time no gas venting in to the atmosphere. The pilot light in the flair tube is shut off. I talked to Randy (MFC) he told me that they are going to shut the well in until they can get a sails line put in. then they will sell the gas.

| On site by request of Tom Tugend.  The Division received a call on 11-03-05 regarding a blow out. When I arrived on the site I found that  Randy Mathiney, a rig crew and a Halliburton crew were at the site also. They were in the presses of putting acid in the well.  They had just removed the frac valve and put a well head on the casing. The well pressured up and blew fluid up in to the air approx 25 foot the wind caught the fluid and blew a mist out across the field to the north and on to a passing vehicle. The local fire dept was called and responded to the site. the rig crew put the frac valve back on the casing and Halliburton started pumping fluid in to the well to kill it and balance out the pressure so that they could run tubbing and put the well in to production. A small amount of spent acid was on the ground around the well head. Randy had already  made plans to have the site cleaned up with vac trucks.

3:36 pm I phoned and talked to Jim Aslanides he is going to contact the pumper and find out if the separator has been bypassed and put the gas back to the flair. then call me and let me know what was the problem. As I was leaving the site the flair came on. Indicating that the separator is working.

-1659305415 2006-05-17 On site as the result of a phone conversation w/ Fred Jackson (614) 530-3459. 5-17-06 at approx 9:04 Am I received a call from Mr Jackson he wanted to know if flaring gas was pollution. I told him no and that burning a flair was legal in Ohio if there was no market for gas. He also wanted to know about fire hazards form burning the flair. Such as trees, I told him he would have to talk to the local fire department. He got mad and said that he appreciates how much his government is doing for him. Then he hung up on me.

The well is shut in at this time no gas venting in to the atmosphere. The pilot light in the flair tube is shut off. I talked to Randy (MFC) he told me that they are going to shut the well in until they can get a sails line put in. then they will sell the gas.
