The tank battery and wellhead are in separate 8-foot board-on-board enclosures. The well is currently in production.  The  spacing within the tank battery and between the unit elemetns is in accordance with 1509:9-9-05 . Unit identification is present and legible at the tank battery. The tank battery secondary containment is adequate.  I did not observe any leaks, spills, or other signs of contamination. The gates are locked.

The wellhead and battery are located near the rear portion of a church property.  The wellhead plunger lift and battery are in separate board-on-board enclosures which are locked. Fittings and connections are secure and free from signs of leakage or problems. The load-outs are secure, plugged, and locked. Process safety gear is present. Signage is in place and current. At the time of this inspection there are 15 to 20 inches of snow cover in the area which obscures some surficial elements from observation.

| On site for long string cement job.  Hole #3  7.875 inches in diameter, total depth 3510 feet.  4.5 inch casing set at 3447.50 casing was new R-3 11.6 lb pipe.  Top of the Lockport was 2812'.  Cement fill up needed 798 feet, 798 X .2278 / 1.61= 112.9 sacks of cement.  Operator ran 200 sacks of Unitropic 10-4-2 cement.  Operator broke circulation and conditioned the hole with 5 joints of pipe remaining to be ran.  Operator completed running pipe, broke circulation and ran cement. Operator displaced cement and locked down plug with 1500psi.  No circulation issues during displacement.
