WCS washed in 1 ton of clay from 1153 feet to 1091 feet and began to rip the 8.625 inch casing.

| The well has been plugged by Big Sky Energy. Upon taking equipment out of the location the land owner Dan Mercer called the County Highway Department which initiated the complaint of 4709. Big Sky was pulling a tractor and trailer loaded with pipe and tubing with a dozer and pushing with a dozer up the grade and on the road. The chip and seal was tore up leaving the section of road soft. The Highway department called me and they would fix the road and send the bill to the company.

| Per the request of Legal Section I performed a Final Restoration inspection on plugged well site. I took pictures from start to finish. There is a patch job on the road from the County Highway department due to Big Sky running dozer on chip and seal and pulling equipment out. The road down the hill between the two lanowners has been put back by the landowners. They used old crushed blacktop was used as a compaction material. Big Sky did make ditch for them on the west side down to the fence. The first gate was hit by Big Sky equipment. They never fixed it back to original status. The second gate is where the Property is owned by  State of Ohio Division of Wildlife. From that point on the lease road is chopped up and wet. The mud is knee deep. There was orange pipe in the road . Upon well site there was trash and material left. There were items such as a piece of 8.625''casing, 2'' white plastic flow pipe, 8.625''cast plug, wire and metal. There was a tool left from rig and 1''orange plastic pipe. The company plugging the well was specifically told to remove all material before leaving the site. The Final Restoration has not passed at this point. Remove all material and install property back per Soil and Water Best Mangement Practices.

When I arrived on location we had to shut the service rig crew down. They did not have the right pre-cast plug for the plugging job and they did not have there 8" tools to rip the pipe. They are going to get the right stuff and come back on Monday.

WCS continued plugging well with clay when the hole filled up with water. At that time a decision was made to run tubing and plug out with cement.

| I stopped by location at approximately 1200 hrs to 1300 hrs to verify that the 1''orange tubing was removed from the site. The company did not remove the tubing at this point. I called Roger Barr and he would have a crew out at about 2:30 pm.

WCS left location for a fishing tool for retrieval and will begin fishing tools out tomorrow.

I witnessed that Well Completion Services pulled 12 joints of 4.5 inch casing and then washed a 285 foot Big Lime clay plug from 2650 feet to 2935 feet. WCS pulled 1 joint of 4.5 inch casing and their power tongs broke. Shut down to get replacement parts for tongs.

Formation Cementing Services pumped a 200 foot class A Berea plug from 1066 feet to 866 feet, WCS pulled 9 joints of 1.5 inch tubing, Formation Cementing Services pumped a 200 foot class A bottom of the surface casing plug from 765 feet to 565 feet, WCS pulled 16 joints of 1.5 inch tubing and Formation Cementing Services pumped a 240 foot surface plug from 240 feet to surface. WCS pulled last 7 joints of tubing and cement stayed to surface.
1402311007 2014-06-07 Well has been plugged but the final has not been completed. There is wire and piping laying around that needs removed. The dirt work needs to be completed on the wildlife property.
1403884112 2014-06-27 Per the request of Legal Section I performed a Final Restoration inspection on plugged well site. I took pictures from start to finish. There is a patch job on the road from the County Highway department due to Big Sky running dozer on chip and seal and pulling equipment out. The road down the hill between the two lanowners has been put back by the landowners. They used old crushed blacktop was used as a compaction material. Big Sky did make ditch for them on the west side down to the fence. The first gate was hit by Big Sky equipment. They never fixed it back to original status. The second gate is where the Property is owned by State of Ohio Division of Wildlife. From that point on the lease road is chopped up and wet. The mud is knee deep. There was orange pipe in the road . Upon well site there was trash and material left. There were items such as a piece of 8.625casing, 2 white plastic flow pipe, 8.625cast plug, wire and metal. There was a tool left from rig and 1orange plastic pipe. The company plugging the well was specifically told to remove all material before leaving the site. The Final Restoration has not passed at this point. Remove all material and install property back per Soil and Water Best Mangement Practices.

Upon taking equipment out of the location the land owner Dan Mercer called the County Highway Department which initiated the complaint of 4709. Big Sky was pulling a tractor and trailer loaded with pipe and tubing with a dozer and pushing with a dozer up the grade and on the road. The chip and seal was tore up leaving the section of road soft. The Highway department called me and they would fix the road and send the bill to the company.
