| I met OEPA and Corp Oil Field services on site and found all oil has been cleaned out of the creek. OEPA instructed Corp services to remove all containment devices from the creek. There is still an unknown amout of oil recovered because the oil is settling out of the water. I will contact Corp Oil field services at later date. Complaince Notice is resolved on 5-5-14 by Patrick Shreve.

| At 14:30 on 4-16-14 Bill Arnholt with Northwood Energy called on an Oil spill of unknown amount. Upon ODNR arrival at well site I found that the oil spill was very large. There was 71.6 barrels of Clinton crude that was spilled. There was an old buried flow line that went from dike down over hill. The tank busted out at the bottom due to bad weld and allowed the oil to penetrate the old flow line and escape.  The rest of the oil was coming from the tank dike as it went over and down the hill about 75 yards where it found a field drain tile. The oil went down about 300 yards down toward creek and surfaced out of the ground.  From there about another 200 yards away the oil was captured by a brush dam in creek. There it contained about 4-5 barrel. So far approximately 55-60  barrels have been recovered. There were some plugs not in place on the day of the incident. Trevor Irwin with OEPA has been out and working on cleanup with Company and ODNR. The absorbent booms will be left out for a few days for any minor retention

| I stopped by to Look at the well site. All problem areas have been vegetated back and the tank dike is dry and free of water. This resolves the compliance notice # 1397762598.

| At approximately 1:20 om on 3-13-15 Trevor Irwin with OEPA called on a well that had a possible contamination by digging open pit and cleaning residue out of pipes and then covering up. Upon inspection the following was revealed: There was a service rig at the well at some point and time. They had dug an open pit and steamed the 2.375'' tubing out. There was no liner and the paraffin was placed in the open pit. Then the company covered over the pit. They placed peat absorbent over the areas with some residual paraffin. The area was approximately 10'x20'. The well was put back together and placed into production. I called Mike Corp the service contractor and Bill Arnholt with Northwood Energy. They said when it dries up they will bring equipment and remove all paraffin and debris. I called back Trevor Irwin with the OEPA and told him the plan. I stopped up and talked to the landowner and told him it would be cleaned up as soon as it dries up and I will be on site when the work takes place. I took a few pictures for my records. There is no sheen at present time.

| Mike Corp had equipment out on location to clean up the area where there was parafin and clean out the pit down to virgin soil. There was a track-hoe and a quad- axle dump truck. There was about 17 ton of contaminated soil. I will have tickets sent to me. The truck was stuck and the Dozer was called out to extract the truck out. The truck left ruts but will tended to when the weather dries up.

| Bill Arholt with Northwood Energy called me back on the Tank site. The pumper will keep dike site pumped out and monitor the oil coming out of the soil. The tanks were not leaking. I told Bill to keep removing the oil with absorbent pads.

1398109474 2014-04-21 I met OEPA at the site and we walked up and down the creek. There were absorbent booms and diapers in the creek below the final Collection point. OEPA wants to adjust the orange floating impoundment and change the oil laded absorbent booms out. The decision was made to remove the old flowline. The company made a catch basin and was removing the hydrocarbon down stream. The company peeled back the ground that was letting the oil seep out of the ground. They collected the oil and placed in the tanker truck. They then covered back up. The company had Corp oil field service install tanks and plumb up to the well.
1399284359 2014-04-22 I met with OEPA and Corp oilfield services at well site and went down the creek toward Licking county line cleaning pockets of oil out of creek. I took pictures and coordinates at various intervals in the Valley Run creek. Monitor Creek and change out absorbent booms and diapers.
1416592116 2014-11-21 I stopped by to Look at the well site. All problem areas have been vegetated back and the tank dike is dry and free of water. This resolves the compliance notice # 1397762598.
1426502691 2015-03-13 At approximately 1:20 om on 3-13-15 Trevor Irwin with OEPA called on a well that had a possible contamination by digging open pit and cleaning residue out of pipes and then covering up. Upon inspection the following was revealed: There was a service rig at the well at some point and time. They had dug an open pit and steamed the 2.375 tubing out. There was no liner and the paraffin was placed in the open pit. Then the company covered over the pit. They placed peat absorbent over the areas with some residual paraffin. The area was approximately 10'x20'. The well was put back together and placed into production. I called Mike Corp the service contractor and Bill Arnholt with Northwood Energy. They said when it dries up they will bring equipment and remove all paraffin and debris. I called back Trevor Irwin with the OEPA and told him the plan. I stopped up and talked to the landowner and told him it would be cleaned up as soon as it dries up and I will be on site when the work takes place. I took a few pictures for my records. There is no sheen at present time.
1531049444 2015-03-23 Mike Corp had equipment out on location to clean up the area where there was parafin and clean out the pit down to virgin soil. There was a track-hoe and a quad- axle dump truck. There was about 17 ton of contaminated soil. I will have tickets sent to me. The truck was stuck and the Dozer was called out to extract the truck out. The truck left ruts but will tended to when the weather dries up.
1666038054 2014-06-24 On the day of inspection I accompanied Pat Shreve to the well site to follow up on a previous contamination issue at the well tanks. Refer to Pat Shreve's report for further details.
1704928850 2014-06-26 I checked the tank site and the water has been sucked out of the dike. There is oil in the dike. The company is aware of the issue. I tried calling Bill Arholt with Northwood Energy and Mike Corp. I did not get a response.

2041877481 2014-06-27 Bill Arholt with Northwood Energy called me back on the Tank site. The pumper will keep dike site pumped out and monitor the oil coming out of the soil. The tanks were not leaking. I told Bill to keep removing the oil with absorbent pads.
286479398 2013-01-07 On day of my inspection the following revealed there was a pumping unit, Two steel one hundred barrel tanks, dike and id. Well was producing. Annular disposal was not hooked up.
-354203635 2014-05-05 I met OEPA and Corp Oil Field services on site and found all oil has been cleaned out of the creek. OEPA instructed Corp services to remove all containment devices from the creek. There is still an unknown amout of oil recovered because the oil is settling out of the water. I will contact Corp Oil field services at later date. Complaince Notice is resolved on 5-5-14 by Patrick Shreve.
