| I performed a random field inspection.  I GPS'd both the tank battery and the wellhead.  I continued to walk around both the wellhead and the tank battery.  This inspection satisfies 1501:9-9-05.  There were no violations at this time.

| Arrived on site and met with Russell Miller. On January 25, 2006 for unknown reasons the 210 bbl storage tank in the tank battery exploded and launched 250' to the south. The tank contained approximately 60 bbls of fluid in it at the time of the explosion. Fourty barrels of fluid was recovered from the dike. The other 20 bbls of fluid was within 100 feet of the dike to the south. Because oif the potential to enter a waterway 100' north the company was reminded of their responsibility to contact OEPA. The tank battery was destroyed. Russell was informed that since the tank battery has to be completely rebuilt and it is located in an urbanized area that it had to be built to new standards. These standards were discussed and a copy of the will be provided to him. Case pends.
