| A valve broke (froze) on 1/20/14 and 80 bbls of brine and a small amount of oil were lost.  The dike around the tank and the sump caught most of the brine and all of the oil. No oil left the dikes.  Approximately 10 bbls of brine escaped.  The brine inside the dikes and sump have been removed and placed back in the tanks.  The valve has been replaced.  The residual oil has been covered with Oil Gator.  The area where the brine escaped has been covered with lime.  Further remediation will resume when the weather breaks.  The annulus of the disposal well has had 120 psig put on it.  An annulus valve that was found to be leaking has been replaced.

| During my inspection I found a brine kill area below the pumphouse vault. The area is 15 ft wide and 50 ft long. The grass and vegetation in that section has been burned and is dead. I do not see a point of origin for the brine . It may have been an over flow of the vault . It needs excavated and removed and clean fill put back in its place. Next the area should be seeded and mulched for cover until vegetation can be established .

| FOLLOW UP TO NOV #  -1926063431 During my inspection I found a brine kill area below the pumphouse vault. The area is 15 ft wide and 50 ft long. The grass and vegetation in that section has been burned and is dead. I do not see a point of origin for the brine . It may have been an over flow of the vault . It needs excavated and removed and clean fill put back in its place. Next the area should be seeded and mulched for cover until vegetation can be established .

The spill area will be evaluated and the contaminated soil removed as necessary. The brine that escaped was mostly Berea water.

-409792556 2014-01-21 A valve broke (froze) on 1/20/14 and 80 bbls of brine and a small amount of oil were lost. The dike around the tank and the sump caught most of the brine and all of the oil. No oil left the dikes. Approximately 10 bbls of brine escaped. The brine inside the dikes and sump have been removed and placed back in the tanks. The valve has been replaced. The residual oil has been covered with Oil Gator. The area where the brine escaped has been covered with lime. Further remediation will resume when the weather breaks. The annulus of the disposal well has had 120 psig put on it. An annulus valve that was found to be leaking has been replaced.
