| This inspection was done at the request of the landowner Norman Liebrecht and Henry County EMA Director Tracy Busch. At approximately 11pm on Monday, May 26th, the landowner's son was spraying an agricultural field owned by Liebrecht Farms. During the spraying of the field the spray boom made contact with the well and broke a 2.5" collar between the wellhead and the 2" valve holding all gas pressure in the well. This caused a release of gas from the well bore that could not be stopped. The Henry County EMA along with New Bavaria Fire Department (NBFD) were on location around midnight. Duct tape and hose clamps were put on by the NBFD in an attempt to stop the leak. The leak was suppressed but full control of the release could not be maintained. I was contacted by EMA officials at 12:50 pm on Tuesday, May 27th, and was on location at 2:15 pm the same day. Using the state issued gas meter gas levels were monitored at the leak and around the perimeter well for three hours. Gas Levels at the leak maxed at 11ppm Hydrogen Sulfide (H2S), LEL 37, Methane 2.25, and CO 23. A slight smell of sour gas was evident downwind from the well but no gas was detected in any levels except at the leak. The winds at that time were out of the west at 15 mph or greater. Second Oil Company was contacted and will be on location tomorrow to discuss options for containing the leak. The well is located on CR 6 between SR 108 and CR 13. This part of the road was closed by Henry County EMA and will remain closed until further notice.

| At the time of the inspection I was on location with the landowner Norman Liebrecht, Henry County EMA Director Tracy Busch, and Tony Kryder of Second Oil Company. Using the state issued gas meter, gas levels were monitored at the leak and around the perimeter well for one hour. Gas Levels at the leak maxed at 45ppm Hydrogen Sulfide (H2S), LEL maxed out, Methane 1.25, and CO 20. A slight smell of sour gas was evident downwind from the well but no gas was detected in any levels except at the leak. The winds at that time were out of the east at 5 mph. The landowner excavated around the base of the well and a 36" diameter, 15' tall, corrugated plastic riser was placed over the well. This will allow any escaping gas to vent above ground level and dissipate. The well will be monitored by ODNR and Henry County EMA until the leak can be fixed and the well plugged.

| At the time of the inspection the well was still leaking gas inside the temporary riser placed over the well. A slight smell of sour gas was evident downwind from the well but no gas was detected in any levels. There was no evidence of any fluid leaking from the well or around the riser. Conditions were rainy with winds out of the east at 10 mph.
