| On  the day of my inspection Bakerwell released the spear and tripped the jars out of the hole. They went back in with a 5 mill in an attempt to clear any obstructions that my be preventing the packer from coming up the hole.

| On site to pressure test the integrity of the 7" casing, rig crew ran a mechanical bridge plug too 5872' and  they then filled casing with fluid and they closed the Blow Out Preventer and then they applied 1450 psi of pressure and we monitored pressure for 30 minutes with no pressure loss, then decision was made to pull bridge plug and then shut well in for the evening they will resume working on changing out well head in the morning before running down hole magnet in well to try and retrieve lost packer slip assembly.

Note: Prior to getting line up to pressure they had to tighten all connections they had five leaks prior to getting well tested, company rep stated that injection pump sales rep would be on site in the near future to test pump prior too placing it into operation.

1350775643 2012-10-18 Inspection revealed that injection pump was not in operation during my inspection and we pressure tested flowline to well head@1510 psi and we monitored pressure for 15 min. with no decline in pressure(Test Passed) and there was a Infinity Vacuum truckUIC#486 on unloading pad it took 13 minutes to unload truck into holding tanks.

Note: Once all tanks are full they will start pump and check plumbing for possible leaks.

1358984966 2013-01-23 On site to witness service rig try and release packer they tried several different ways to release packer with no luck, decision was made to part tubing just above packer and then attempt to fish packer from well using tubing and washover assembly.

1359139742 2013-01-23 On the day of my inspection I met ODNR Inspector Dave Ball on location to witness Bakerwell attempting to mechanically release the packer. It was reported that annular pressure was detected, requiring that the casing and packer be removed and tested.
1359202377 2013-01-25 Inspection revealed that service rig has removed 4.5 injection string from well they will nipple up BOP and then run 2.375 tubing with fishing tool on bottom and then proceed to go in well to try and retrieve packer from well.
1359503003 2013-01-28 Inspection revealed that service rig had to mix kill water before starting out of hole, pulled tubing and Blue-Dot tools from well and we did not retrieve packer at this point, decision was made to go in hole with down hole sphere and a set of hydraulic jars and try and retrieve lost packer.

1359644793 2013-01-30 On the day of my inspection Bakerwell released the spear and tripped the jars out of the hole. They went back in with a 5 mill in an attempt to clear any obstructions that my be preventing the packer from coming up the hole.
1359751862 2013-02-01 Inspection revealed that service rig has packer moving up hole but it is still dragging a little it will be a slow process before they get packer out of hole, once out of hole they plan on running a casing scrapper in the well to make sure there is no burrs on inside of casing and then we will start new packer in well .
1360110387 2013-02-04 On site to pressure test the integrity of the 7 casing, rig crew ran a mechanical bridge plug too 5872' and they then filled casing with fluid and they closed the Blow Out Preventer and then they applied 1450 psi of pressure and we monitored pressure for 30 minutes with no pressure loss, then decision was made to pull bridge plug and then shut well in for the evening they will resume working on changing out well head in the morning before running down hole magnet in well to try and retrieve lost packer slip assembly.
1360155167 2013-02-05 On site to witness Arrow Set Lock packer in hole, before running packer decision was made to run magnet in well to retrieve broken packer parts we got small amount of metal we made three runs with magnet and then ran bare tools in well to check total depth@7241' and then they proceeded to change out well head on well head was in place it was determined that packer would not go through it then they had to call in well head personnel to grind out obstruction then we started packer in hole.
1360249979 2013-02-06 On site to witness 4.5 injection string &packer run in hole and then pressure test annulus of tubing,packer was set under 20K tension and well head used as a mandrel type and a sphere will need to be used if packer is to be released at a later date.Universal Well Services preformed pressure testing and pumping of 117 bbl of fresh water with inhibitor on backside of 4.5 casing and once testing was complete they had to have welder replumb flowline to well head and then we retested flowline @1500 psi and no leaks were detected at this time.
