| I arrived on this site today for a routine inspection. I found that this well is idle. During a records check of production, it was determined that this well has been idle since 1998. I have contacted the Operator and advised that this well needs to be plugged. The option to produce as part of a plug or produce order will not be included. All of the equipment needed to produce this well is not in place. The Operator must apply for a plug permit and continue with the plugging of this well in a  90 day period.

| Start plugging this well at 3898 ft and set a 400 ft plug at 3898 to 3498 ft. Pull 1-1/2 tubing out and then pull 4-1/2 inch casing out. Recovered 1957 ft of casing then ran back in well with 1-1/2 inch tubing to 1700 ft. Set plugs # 2 and 3 which were combined to make a 400 ft plug from 1700 ft to 1300 ft which is a back up plug and the bottom of the surface plug. Then use the tubing to set the last 200ft to surface. Had good cement returns to surface and plug remained at surface after  the job was complete.

Casing can be cut off below ground level and a cap with the permit number placed on the well.

1354545192 2012-11-27 I arrived on this site today for a routine inspection.
1383314491 2013-10-31 Start plugging this well at 3898 ft and set a 400 ft plug at 3898 to 3498 ft. 1790155487 2013-11-04 This well has been plugged to surface. I am on site today to check for leaks and to see if the cement remains at surface. During my inspection I found that the cement is still at surface and that there are no gas leaks at this time. Casing can be cut off below ground level and a cap with the permit number placed on the well.
