| My inspection today found the area contaminated by oil in the creek bed had been cleaned up. The hole in the flow line was temporarily repaired by a pipe clamp. The flow line riser in the tank battery was not repaired and there was still peat-sorb spread around the area. Petrox has Scott Miller Construction scheduled to make the repairs and clean up the rest of the tank battery.

| I was called to the location by Geauga Emergency Services Ken Folsom for a flow line leak and about 20 gallons of oil on the ground. My inspection found the flow line had two holes in it. One in the tank battery where the flow line riser comes out of the ground and the other in a dried up creek bed where the flow line was run above ground. The landowner had previously lined the creek bed by his road culverts with cement to prevent erosion. The oil was laying on top of the cement. The company had absorbed the majority of the oil with absorbent mats and peat-sorb. The company was to return in the morning to clean up the rest of the contaminated areas. The lines are scheduled to be repaired on Monday the 9th of June.

| My inspection found the well to be equipped with a plunger lift unit. The wellhead was surrounded by a chain link fence. The tank battery is surrounded by a chain link fence and a sufficient dike. The tank battery consists of a 100 bbl production tank and a mechanical separator. The identification is on the tank battery. The lease road is in stable condition.
