The well is pumping  by a green and yellow painted Continental Emsco electric pump jack and the well pad is free of excess contamination. The storage consists of two silver tanks and two vertical separators. The storage is shared with the Bible #3 (#2587). The well identification for both wells and emergency contact information is marked at the storage. Status OK.

| On site by request of Garry VanDyke (pumper). Approx 12:30 AM the neighbor noticed the odor of oil and called Garry. He went to the site and found that the flow line had ruptured and let an undetermined amount of  oil out on to the ground. The oil went to the road ditch and then south approx 50 foot into a culvert and under Co Rd 24, a dam was installed at that point by Mr.Howerd (neighbor). I was notified at approx 1:00 AM. Arrived on the site at 1:22 AM, a vac truck was on site and clean up had been started. The bulk of the free oil was removed w/the vac truck and the shut down untill day light to further asses the situation for further clean up.

1186725415 2007-08-10 On site by request of Garry VanDyke (pumper). Approx 12:30 AM the neighbor noticed the odor of oil and called Garry. He went to the site and found that the flow line had ruptured and let an undetermined amount of oil out on to the ground. The oil went to the road ditch and then south approx 50 foot into a culvert and under Co Rd 24, a dam was installed at that point by Mr.Howerd (neighbor). I was notified at approx 1:00 AM. Arrived on the site at 1:22 AM, a vac truck was on site and clean up had been started. The bulk of the free oil was removed w/the vac truck and the shut down untill day light to further asses the situation for further clean up.
1210i1056839 2002-03-20 Well is pumping. Earthen dike has had some more material scraped into place. Right now dike is only about 75% complete. Oil soaked straw still on ground and free oil visible. Took 5 more photos. Clean up is not acceptable.

The well identification for both wells and emergency contact information is marked at the storage. Status OK.
