| On 8/28/2012, I completed an urban production well inspection status check.  My inspection found the well equipped with a plunger lift unit (rabbit) that was leaking brine and oil.  The contaminated area was approximately 3' in diameter.  I called the well tenders, Strawn Oil Service, at 330-537-3863 and asked for the leak to be cleaned up by 9/3/2012.  The tank battery contained one 100 barrel oil storage tank, one 50 barrel brine storage tank, a separator, and the proper identification posted.  The lease road was in good condition.
