A 1' x 6" trench cut out along backside of dike for drainage(2) 210 bbl tanks are corroding, but unable to tell if leaking due to fluid inside dike.  The ID at tank location contains only correct owner name and emergency contact#..  The dike height is from 6' to 7'.  The liner contains approximately 5 tears along the top backside of dike.  The tears range from 6" to 2'.  Positive pressure was observed on annulus.

The unloading pad also needs to be repaired because, it is cracking. No identification at the wellhead or at the storage tanks.

A section along the side of this dike contains a trench and will need to be repaired. The company has been instructed of this deficiency to include the liner that contains small tears along the top. The identification at both the well and tank battery locations have been painted over incorrect well owner, Mountaineer Gas Transmission. Adams Oil & Gas and ERP # will need to be added to the ID.

No identification was observed at the well and storage tanks. A trench cut in the side of the dike will need to be repaired. The liner contains small tears at locations along the top of the dike.

A 1' x 6 trench cut out along backside of dike for drainage. (2) 210 bbl tanks are corroding, but unable to tell if leaking due to fluid inside dike. The ID at tank location contains only correct owner name and emergency contact#.. The dike height is from 6' to 7'. The liner contains approximately 5 tears along the top backside of dike. The tears range from 6 to 2'. Positive pressure was observed on annulus.
