| Compliance Notice Follow-up 1740194305: Contamination has been removed from around the wellhead and placed in a nearby area for bioremediation. Dan Miller has built a small dike area and limed the oil and brine contamianted soil. He has also seeded thrown down straw all around the wellhead and the area is already growing grass. He has also removed all of the drums and cleaned out the plastic tank.

| I was out here on 3/13/2014 and reported that the well was not found at the RBDMS coordinates and was a 7" casing sticking out of ground by the pond at Dan Miller's house. Dan told me that this was in fact an old water well. Dan Miller, who rents the house from the landowner, showed me the correct location of the well. It is currently idle and is equipped with a pumpjack. There is no motor. There are eleven full 55 gallon drums and one small plastic tank adjacent to the wellhead. There is contamination and scald area from the plastic tank which is flowing over every time it rains. It is pretty far back to the wellhead and the ground is too wet at this time to take care of the issues. The well is located just above Ron Downing's house and he stated he wants to take over this well and well 21928 for domestic use. He said when the weather get better he plans on getting a service rig or someone out to fix issues down hole. Operator must remove all contamination and restore vegetative growth.
