| On 9/20/2012, I was on the above location to attend the prespud meeting conducted by Wendell Shrewsbury Drilling Manager and Jack Black Compliance Supervisor. After the meeting we walked the perimeter of the location and inspected all of the containment around the drill pad this location is in a flood plain the pad drains to the southwest corner of the location. C N X had 4 skids of large sand bags delivered to the location to be used if it breeches.

| On 9/26/2012, I was on location to witness the cement the 20" conductor casing to a depth 374', as they were running the casing it would not drop past 240' ( 6 joints of casing). They hooked up there pumps and tried to circulate down and around the casing to get it to drop after several attempts the decision was made to  trip the casing back out and run back in with a Tri-Cone bit and clean the well bore.

| I was on site today with Inspector John Fleming and Deputy Chief Bob Worstall.  Calfrac was on site rigging up to fracture this well.  I walked the site with company personnel.  All fracturing chemicals where on secondary containment.  I reviewed the chemicals on location with staff.  Bulk sand units are on location.  I discussed respiratory precautions when dealing with silica sand.  I was informed staff that will be working with the bulk units will have respirators.  Staff outside the limits of the bulk units and within the OSHA safe area will not have respirators.  I was also informed that these are older units.  Industry is moving towards using a conveyer system that will limit dust.  These units are air units.  Frac iron was restrained with Kevlar flow line safety restraints rated at 20,000psi.  All frac iron on site is rated well above any pressures that are expected to be encountered.  Frac iron was pressure tested during inspection, and passed.  Frac stack was in place on the well. Frac stack is rated for an operating pressure of 10,000psi.  Frac stack had two manual shut off valves and a set of hydraulic rams.  Safety shut downs were in place.  Anticipated pressure is +/- 4,100psi.

We discussed the installation of an in ground vault at the southwest corner of the pad to collect all of the run off water and  any contaminates if released would be collected in the vault and vacuumed up by a water truck that is on location at all times during the frac operations.

| I performed a status check on the above location. At present this well is still shut in. A sales line has been installed. This well shares the same tanks as the MAHN7CAHSU 340993184. There is six 400 barrel tanks and one GPU unit and a VDU unit to burn off tank vapors. Pettigrew Pumping is the company producing the well for CNX. There phone number is 330-297-7900. There is various emergency shut downs on the pad.

At this time they are installing 8000' of 2 steel sale line to a Dominion East Ohio pipeline. This is a DOT regulated line. A large part of this line was bored using fresh water only. There is a lot of construction mats in place. There is also erosion and sediment controls in place. When pipeline crew and equipment leave location I will inspect the well and tank battery location.

| I was on location with Inspector John Fleming.  I walked location; operator was rigging up for fracturing procedures.  Operator installed the catch basin in the south west corner of the location.  I had been on location last week to meet with the operator in regards to a proposed catch basin.  The catch basin was installed.  The catch basin is in case there was a release that got past the primary and secondary containment on location.  The catch basing is controlling all drainage coming off of the location were the equipment is located at.  The operator has a sump pump in the basin allowing for the controlled release of fresh water.  Sediment and drainage controls are in place.  Water coming off of the site was running clear.

No violations noted at this time. There are currently drilling in the Mt Pleasant at a depth of 12600'.

We walked the area he explained to me the operation of the new equipment that they were installing. Fire Extinguisher are in place. All ID signs are posted. No violation were noted at this time.

1323013527 2012-09-20 On 9/20/2012, I was on the above location to attend the prespud meeting conducted by Wendell Shrewsbury Drilling Manager and Jack Black Compliance Supervisor. After the meeting we walked the perimeter of the location and inspected all of the containment around the drill pad this location is in a flood plain the pad drains to the southwest corner of the location. C N X had 4 skids of large sand bags delivered to the location to be used if it breeches.
1349193309 2012-09-26 On 9/26/2012, I was on location to witness the cement the 20 conductor casing to a depth 374', as they were running the casing it would not drop past 240' ( 6 joints of casing). They hooked up there pumps and tried to circulate down and around the casing to get it to drop after several attempts the decision was made to trip the casing back out and run back in with a Tri-Cone bit and clean the well bore.
1351691417 2012-10-31 I performed a status check on the above location. I walked the perimeter of the drilling rig location and inspected for contaminants. All rainwater has been contained on the pad. Clear Creek has been on location vacuuming all water from the drill pad and disposing of it at the SRI facility. I also inspected the Southwest corner of the drilling pad where the water drains to a sediment trap. It is free of contaminants at this time. There has been 2-3 of rainfall in the past 48 hours.
1353342364 2012-11-17 Nabors M-62 drilled to a total depth of 5700'. Ran 123 joint of 9.625 40# P-110 BTC casing to a depth of 5614' KB. Halliburton on location to perform a 2 stage cement job, a total of 780 sacks of HALCEM and 1005 sacks VARICEM was used during the cement job. Job completed at 6:48 am . 1356104848 2012-12-07 On 127/2012, I was on location an performed a status check at present they are running there 5.5production casing. No violation at this time.
1363362255 2013-03-14 I was on site today with Inspector John Fleming and Deputy Chief Bob Worstall. Calfrac was on site rigging up to fracture this well. I walked the site with company personnel. All fracturing chemicals where on secondary containment. I reviewed the chemicals on location with staff. Bulk sand units are on location. I discussed respiratory precautions when dealing with silica sand. I was informed staff that will be working with the bulk units will have respirators. Staff outside the limits of the bulk units and within the OSHA safe area will not have respirators. I was also informed that these are older units. Industry is moving towards using a conveyer system that will limit dust. These units are air units. Frac iron was restrained with Kevlar flow line safety restraints rated at 20,000psi. All frac iron on site is rated well above any pressures that are expected to be encountered. Frac iron was pressure tested during inspection, and passed. Frac stack was in place on the well. Frac stack is rated for an operating pressure of 10,000psi. Frac stack had two manual shut off valves and a set of hydraulic rams. Safety shut downs were in place. Anticipated pressure is +/- 4,100psi.
1363964159 2013-03-04 Northeast Region Supervisor Tom Hill and myself met with Bob Dejaegher CNX Construction Supervisor at the above location. We discussed the installation of an in ground vault at the southwest corner of the pad to collect all of the run off water and any contaminates if released would be collected in the vault and vacuumed up by a water truck that is on location at all times during the frac operations.

The location is clean of debris, all safety signs are posted and fire extinguish are in all work areas. No violations were noted at this time.

This well shares the same tanks as the MAHN7CAHSU 340993184. There is six 400 barrel tanks and one GPU unit and a VDU unit to burn off tank vapors. Pettigrew Pumping is the company producing the well for CNX. There phone number is 330-297-7900. There is various emergency shut downs on the pad.
1483950578 2014-12-16 I meet with Ted Meyers Gathering Supervisor for CNX and Nicholas Oswalt Area Manager for Integrity Kokosing Pipeline Service. At this time they are installing 8000' of 2 steel sale line to a Dominion East Ohio pipeline. This is a DOT regulated line. A large part of this line was bored using fresh water only. There is a lot of construction mats in place. There is also erosion and sediment controls in place. When pipeline crew and equipment leave location I will inspect the well and tank battery location.

2134681549 2012-08-21 I was on location to meet with CNX and Express Energy personal. They are starting to drill the Cellar and 42 hole for the 30 conductor casing. They are also gone to drill the mouse hole.
313225266 2013-03-13 I was on location with Inspector John Fleming. I walked location; operator was rigging up for fracturing procedures. Operator installed the catch basin in the south west corner of the location. I had been on location last week to meet with the operator in regards to a proposed catch basin. The catch basin was installed. The catch basin is in case there was a release that got past the primary and secondary containment on location. The catch basing is controlling all drainage coming off of the location were the equipment is located at. The operator has a sump pump in the basin allowing for the controlled release of fresh water. Sediment and drainage controls are in place. Water coming off of the site was running clear.

We talked with Courtney Pettit from CNX, he said that they were in the process of getting CalFrac on location to start setting up to start fracing on the week of 3/11/2013.
