| I received a complaint from landowner, Jeff Bond, of an idle well on his property.  I inspected the well site and found the well shut in and the flow lines disconnected.  The one, 100 barrel production tank was empty and the bottom of the tank is showing signs of fatigue.  There has been no production reported on said well for several years.  The well is equipped with 12" conductor pipe, 8.625" surface pipe and 4.5" production casing.  All valves at wellhead were shut on the day of my inspection.  Plug well within 60 days or file for temporary inactive status.  Replace production tank.  Failure to comply may result in bond forfeiture.

| Request from DNR Columbus to inspect wells on the Jeff Bond property.  This well is not in production and not connected to any sales line.  Substantial erosion around the well bore caused by large slip, leaving casing below wellhead exposed for several feet.  Proper identification posted at the tank battery.  The soil dike around the oil storage is in very poor condition and will need rebuilt. There is a large, bowl like feature below the tank battery with no vegetation growing which may be due to soil slippage.  There has been no production reported since 1993 and landowner says he has not received royalties from this well.  Mr. Bond also claimed that a dead spot in his yard, about 100' below the tank battery, was caused by a brine release.  I pointed out that if this was a release from the tank there would have been a kill area from the tank to the spot in his yard.  There has been an excessive amount of rain this season and there is water still laying in this spot.  Also there is no oil related sheen visible on the surface of the water, only an oil-like film from rotting vegetation.  Pictures were taken.  No violation was issued at this time.  The landowner is trying to break the lease.

| Inspection revealed that there has been no change in status of well.  The old storage tank still needs to be removed from site and all valves are still shut at wellhead.  This well has a compliance notice issued on it to plug or produce and if well is plugged erosion issues will be addressed on lease road and well site.  Note: The DNR North Region Supervisor and DNR Hydrogeologist accompanied me during inspection.

| Inspection revealed that landowner has built a fence and gate across lease road blocking well owner from access to well site and also hindering well owner from resolving Compliance Notice issued for leaking tank.  I had requested the well owner to remove damaged tank from site, replace it with a new tank and properly identify it.  They must also report production as required by law.

-1093748823 2011-11-02 I received a complaint from landowner, Jeff Bond, of an idle well on his property. I inspected the well site and found the well shut in and the flow lines disconnected. The one, 100 barrel production tank was empty and the bottom of the tank is showing signs of fatigue. There has been no production reported on said well for several years. The well is equipped with 12 conductor pipe, 8.625 surface pipe and 4.5 production casing. All valves at wellhead were shut on the day of my inspection. Plug well within 60 days or file for temporary inactive status. Replace production tank. Failure to comply may result in bond forfeiture. Compliance Notice

1332374325 2012-03-20 Inspected site along with supervisor and landowners to discuss issues at well site, we talked about erosion problems with lease road and ground slippage below well head it was agreed upon that landowner wants to resolve legal issues with well owner before addressing erosion and slippage issues at well site, well owner has current plug/plug back permit and is awaiting on legal opinion before proceeding with plug back operation.

1421859227 2015-01-21 My inspection today finds the well is shut in and there are no gas transport lines attached to the wellhead. I met the landowner Jeff Bond and he stated that he intends to run a gas line from the well to his house for domestic use within the next couple of months.
-1772876808 2011-12-12 Inspection revealed that there has been no change in status of well. It still remains incapable of production.
-182882168 2011-12-02 Request from DNR Columbus to inspect wells on the Jeff Bond property.
1981816740 2012-10-04 Inspection revealed that there is no change in status of well and owner is waiting on legal system to get lease issues resolved before entering property.
415221286 2012-01-17 Inspection revealed that there has been no change in status of well.
467774616 2011-11-18 Inspection revealed that landowner has built a fence and gate across lease road blocking well owner from access to well site and also hindering well owner from resolving Compliance Notice issued for leaking tank. I had requested the well owner to remove damaged tank from site, replace it with a new tank and properly identify it. They must also report production as required by law.
